Dear Mr. Grimsley:
Pursuant to the Freedom of Information Act (5 U.S.C.
5 552) and the NRC's regulations in 10 C.F.R. Part 9, we hereby request copies of all transcripts, minutes, memoranda, notes, or other documents in any form that purport to quote, paraphrase, or reflect any information obtained by the NRC or any agency or instrumentality of the Federal government from or regarding (a) Mr. Harry Chomiak, formerly Maintenance Foreman, Public Service Electric and Gas Co., (b) Mr. Howard W. Masquelier, formerly Electrical Engineer or Electrical Start-Up Engineer, Westinghouse Electric Corporation, and/or (c) Mr. Robert A. Stokes, formerly Manager of Projects Log-istics, Westinghouse Electric Corporation, concerning an alleged transmittal or delivery in 1974 or any time thereafter of Westinghouse NSD Technical Bulletin 74-1 [or 74-01) and/or Westinghouse NSD Data Letter 74-2 [or 74-02] by Mr. Howard Masqueller to Mr. Harry Chomiak, during the period from Feb-ruary 25, 1983 to the present time, including but not limited to information obtained during the NRC investigation into trip breaker f ailures at the Salem Nuclear Generating Station, Artificial Island, New Jersey, after the trip breaker failures which occurred on February 25, 1983.
This request includes, but is not limited to, documents in any of the NRC's offices in the Washington, D.C.
