The NRC in July 1985, issued amendments to Reactor License No.O-81 and SNM License No. SNM-639 changing the name of the ifcensee from Union Carbide Subsidiary B, Inc. to Cintichem, Inc.
The actual change in license took place on August 1,1985 Because of this change, we request that Cintichem, Inc. be added to the NRC's Quality Assurance Program approval for Radioactive Material Packages and that Union Carbide Corporation's name be deleted.
We also note that the NRC's Approval No. 0109 Revision 1 of Docket 71-0109 for this Quality Assurance Program expires in December of 1985.
We therefore request that this program be renewed and have enclosed new Quality Assurance Program pages.
All pages of the program now show a revision date of October 14, 1985 The program is identical to the ol d approved program except for the substitution of Cintichem's name and minor changes in personnel position titles.
In accordance with 10 CFR 170 " Schedule of Fees" we also enclose a check in the amount of $150 00 to cover the cost of this renewal.
