o NOTICE OF VIOLATION Multigraphics License No. SNM-1310 As a result of the inspection conducted on April 9,1987, and in accordance with the " General Policy and Procedures for the NRC Enforcement Actions, "10 CFR Part 2, Appendix C (1985), the following violations were identified:

1.

License Condition No.12 requires that licensed material shall be used by, or under the supervision of, Bernard Schmidt or Joseph Urban.

Contrary to this requirement, on the day of the inspection, April 9, 1987, it was noted that employees used the material while not being under the supervision of those listed above.

This is a Severity Level IV violation (Supplement VI).

License Condition No. 13A requires each sealed source containing licensed material be tested for leakage and/or contamination at intervals not to exceed six months.

Contrary to this requirement, the sealed sources had not been tested at six month intervals.

Specifically, sealed sources of plutonium-238 (200 milligram in 5 sealed sources) housed in Ohmart source holders, had been tested for leakage and/or contamination on an annual basis.

This is a Severity Level IV violation (Supplement VI).

10 CFR 19.11(a) and (b) require that current copies of Part 19, Part 20, your license, license conditions, documents incorporated into the license, license amendments, and operating procedures be posted, or that a notice describing these documents and where they may be examined, be posted.

10 CFR 19.11(c) requires that Form NRC-3, " Notice to Employees" be posted.

Contrary to these requirements, on the day of the inspection, April 9, 1987, neither the documents nor the notices were posted or maintained.

This is a Severity Level V violation (Supplement VI).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or l

explanation in reply, including for each violation:

(1) corrective action i

taken and the results achieved; (2) corrective action to be taken to avoid l

further violations; and (3) the date when full compliance will be achieved.

l Consideration may be given to extending your response time for good cause shown.
