Un August 28, 1984 Industrial Inspection Industries, Inc. submitted a renewal request for its "'). A. Program Approval for Radioactive Pack-ages".
Incorporated in this renewal was a change of registrant name and mailing address.
Additlunally, Specific liaterial License 34-14071-01 was amended Feb-ruary 26,1985 ( Amendment 14) to change the company name to:
Glitsch Field Services,.Inc.
Subsequent to these amendments, GFS, Inc. has received several "Cer-tificate of Compliance" renewals for packages for which we have re-gistered as a user.
The mail sticker and enclosed Addressee linting indicates that our registration is still under:
