10CFR71.95 Event Report 99-12
Dear Dr. Paperiello:
The following report discusses a condition discovered during the shipment of Model UX-30 packages. The report is required pursuant to 10CFR71.95(c).
On May 26,1999, USEC was informed by Siemens Power Corporation that a September 23,1998, shipment had been received from USEC with the ball lock pins disengaged. These ball lock pins are required by the Certificate of Compliance to be engaged during UX-30 package shipments. The content of the involved shipment was low enriched (4.396%) uranium.
The cause for the ball lock pins becoming disengaged cannot be determined, however, USEC has revised procedure XP4-TE-UH2400, " Shipping and Receiving Large UF Cylinders," to improve reliability. On Septeniber 28,1998, USEC revised the procedure to add a waming note conceming I
ball lock pins and to provide instructions for lubrication. Subsequently, on November 16,1998, the

procedure was revised once again to add a verification step and a checksheet for locking pin verification.
