Dear Mr. Gagliardo:
This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period May 19-22, 1986.
The following response to the items contained in the Notice of Violation is hereby submitted.
Inadequate Training 10 CFR 50.47(B)(15) requires that adequate provisions exist in the emergency plan to ensure that radiological emergency response training is provided to those who may be called on to assist in an emergency.
Section 8.1.1 of the Emergency Plan states, in part, "A training and annual retraining program is in effect to ensure that station personnel who actively participate in emergency situations are familiar with the contents and responses set forth in this RERP.
The Fort St.
Vrain Training Programs Administrative Manual (TPAM), Situation Response Training Program - RERP, delineates specific training... ".

b P-86532 August 22, 1986 Contrary to the above, on May 22, 1986, the NRC inspectors determined that two key persons assigned emergency response functions (corporate emergency director) had not received all of the emergency response training delincated in the TPAM prior to having been assigned to the emergency response organization, as evidenced by the lack of records documenting the completion of required training.
This is a Severity Level IV violation. (Supplement VIII)(267/86-01).
(1) The reason for the-violation if admitted:
This violation resulted from the failure to perform adequate cross-review of the-training lesson plans against the requirements stated in the Fort St. Vrain Radiological Emergency Response Plan (RERP), section and figure 8.1.1, "Speciali7ed Training Matrix".
The personnel in the training department preparing the lessen plans (new employees hired within the last year), and the reviewers.of the lesson plan failed to review that the matrix against the revised lessor plans.
The matrix is intended to identi fy the basic requirements of the emergency 1
preparedness training progran.
(2) The corrective steps which have been taken and the results achieved:
The personnel in the training department preparing the lesson plans, and the reviewers of the lesson plans were reminded of the necessity to ensure that all elements contained in the RERP Specialized Training Matrix are, as a minimum, addressed in the sesson plans. The emergency response personnel affected by this oversight were promptly re-scheduled for the appropriate training elements missed, and all necessary training completed by August 5, 1986.

To ensure that this violation is positively eliminated, the existing lesson plans will be reviewed to ensure that there are no other similar problems with any other emergency response functional arsignments.
In addition, all personnel added to the emergency response organization will have completed adequate training prior to being formally assigned to the Fort St.
Vrain Emergency Response Organization. A formal process of assignment to emergency response positions will be developed to ensure that candidates for the various positions are selected by the Emergency Planning Coordinator with approval of the candidate's supervisors.
A copy of this assignment will be forwarded to the Suoerintendent of Training. Upon successful completion of the required training and appropriate training records update, the Superintendent of Training will provide notice to the Emergency Planning Coordinator to allow the individual to be placed into the emergency response organization.
