Dear Mr. Keppler:

This letter is in response to the inspection conducted by Meccrs.

M. Jordan, J. Bjorgen, R. Kopriva and J. Mueller on June 11 through July 21, 19o0 of activities at LaSalle County Station. Reference (a) indicated that certain activities appeared to be in noncompliance with NRC requirements.

The Commonwealth Edison Campany response to the two Level IV Violations in the Notice of Violation is provided in the enclosure.

ATTACHMENT VIOLATION:

(AIR 373-100-86-02502)

Technical Specification 6.2.A.6 requires that detailed written procedures be prepared, approved, and adhered to for preventive and corrective maintenance operations which could have an effect on the safety of the facility. Licensee procedure LOP-RP-04 requires the installation of a jumper to prevent a Reactor Building Ventilation System isolation while transferring the reactor protection bus power supply.

Contrary to the above, on June 13, 1986, Procedure LOP-RP-04 was found to be inadequate as it prescribed the wrong termination points for the jumper, resulting in a Reactor Building Ventilation System isolation.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED The jumper was installed in the proper location. The isolation signal was reset and the reactor building ventilation system returned to service.

CORRECTIVE ACTION TAKEN TO AVOID FURTHER VIOLATION Station procedure lop-RP-04 has been revised to reflect the use of jumpers with banana plugs at the appropriate termination points.

The terminals used for the jumper have been provided with a banana plug socket. This allows the jumper to be installed without risking a loss of continuity in the associated circuitry, as happened in this event.

Technical Specification 6.2.A.7, requires detailed written procedures for surveillance and testing requirements to be prepared, approved, and adhered to.

Procedure LES-PC-107, " Unit 1 Group 7 Isolation Logic Systems Functional Test", Step F.3.d requires switch IB21H-S19D to be placed in test, to test the Transverse Incore Probe (TIP) system logic.

Contrary to the above, on June 13, 1986 the Unit 1 operator did not place switch IB21H-S19D to the test position while performing LES-PC-107.

CORRECTIVE ACTIGN TAKEN AND RESULTS ACHIEVED The isolation signal was reset, and the Shutdown Cooling System restarted in accordance with approved station procedures, CORRECTIVE ACTION '1 AKEN TO AVOID FURTHER VIOLATION The Operator involved in this event was counselled on the unacceptability of his performance, and the need for increased attention to detail. The need to double and, if necessary, triple check actions before actually operating equipment was stressed. This event has been discussed with all shift personnel, emphasizing this philosophy.

In addition, the station has undertaken a broad program to upgrade performance and eliminate personnel errors. This program is described in detail in C. Reed's letter to J. G. Keppler dated August 4, 1986.
