This' letter'is a request for a proposal from Battelle-Pacific Northwest Laboratory for additional technical assistance to the NRC Region V Office-under contract, FIN B2802.
Accompanying the enclosed NRC Form 173 is a mcdification to the statement of work describing additional tasks which NRC Region V expects to begin within the next thirty to sixty days.
This information.should be used as the basis for preparing a ' proposal. - Form 189 contains the minimum information needed, and will be used for this proposal.. Standard. terms and conditions for NRC i
work, as approved by Headquarters, DOE, apply to these tasks. Your_ proposal should be submitted in six copies within 30 days of receipt of this letter to:

MEETINGS AND TRAVEL NRC Region V will fund only those trips approved in advance and directly applicable to this contract.
FURNISHED MATERIAL NRC Region V will furnish the contractor with information requiring specific review. NRC Region V will also furnish the contractor with licensee names, address, and other information as necessary for the performance of this contract.
REPORTING REQUIREMENTS The contractor shall file the following reports:

Monthly Business Letter Report The contractor shall submit a monthly business letter report by the 15th of each month to the NRC Region V Project Manager with a copy to Arnold Wieder, Division of Resource Management Administration, NRC Region V.
This report shall identify: the title of the project, the FIN, the principal investigator, the period of performance, the reporting period, and will contain the following sections:

Its purpose is to identify the total costs expended during the period and accumulated to date for each task (e.g., by facility or technical report). The format for this section is:

2.
Technical Documentation The contractor shall submit a technical report to the NRC Region V Project Manager within 10 calendar days after the completion of an inspection.
l The report shall identify specific items examined, criteria against which items were evaluated, findings and conclusions concerning compliance with the criteria. A report of work findings is required in an outline and format to be supplied by the NRC Region V Project Manager. The report shall also identify the direct manpower expended for inspection, preparation, travel, on-site efforts, documentation, and an estimate of associated overhead costs.
SUBCONTRACTING The contractor shall notify NRC Region V Deputy Regional Administrator, in writing in advance of entering into any subcontract not contained in the original proposal. This advance notification will include:
1.
A description of services to be called for by the subcontract, 2.
Identification of the proposed subcontractor, 3.
The proposed subcontract costs (in total), and 4.
A statement that the proposed subcontract will not result in a real or apparent conflict of interest situation.
STOP-WORK ORDER t
- 1.
MRC Region V may, at any time, by written order to the DOE Contracting Officer, require the contractor to stop all, or any part, of the work called for by this work order for a period of 90 days after the ordar is delivered to the contractor, and for any further period to which the parties may agree. Any such order shall be specifically identified as a Stop-Work Order issued pursuant to the clause. Upon receipt of such an order, the contractor shall forthwith comply with its terms and take all reasonable steps to minimize the incurrence of cost allocable to the work covered by the order during the period of work stoppage. Within a period of 90 days after a Stop-Work Order is delivered to DOE, or within an extension of that period to which the parties shall have agreed, the issuing office shall either:
a.
Cancel the Stop-Work Order, or b.
Terminate the work covered by this work order.
2.
If a Stop-Work Order issued under this clause is cancelled or the period of the Stop-Work Order or any extension thereof expires, DOE shall authorize its contractor to resume work. An adjustment shall be made in the delivery schedule or cost, or both, and the work order shall be modified in writing accordingly.
O 3.
If a Stop-Work Order is not cancelled and the work covered by such is terminated in accordance with the terms of this work order, costs resulting from the Stop-Work Order shall be allowed in arriving at the termination settlement.
LIMITATION OF FUNDS 1.
The Commission shall not be obligated to reimburse DOE for costs incurred by its contractors in excess of the total amount authorized by an appropriately executed NRC Form 173. NRC Region V will formally notify the appropriate DOE Operations Office of any programs they intend to phase out or terminate as soon as such intent is known: preferably, at least 30 days prior to the proposed termination date. For programs with fixed performance periods, the DOE Operations Office should assume that the program will terminate on the last day of the period specified in the Standard Order for DOE Work unless notified otherwise.
2.
If at any time DOE has reason to believe that the costs will exceed the total amount authorized, DOE shall notify the issuing authority. In the absence of formal NRC instructions to continue or to terminate a program, the DOE Operations Office (contracting officer or his designee) will notify NRC, by TWX or other suitable written means, when the accrued costs of any NRC program approaches 90% of the authorized funding level.
The notification should include the estimated date when the accrued costs will equal the authorized funds, and may, if appropriate, recommend or request the NRC action desired. The notification should be addressed to NRC Region V, with copies to the Director, Office of Resource Management, the Deputy Director, Division of Reactor Safety and Projects, Region V, and a copy to DOE-EP. After such notification, NRC Region V will either:
a.
Increase funding authorization, and/or b.
Change the scope of work, and/or c.
Change the period of performance, or d.
Terminate this work order.
3.
In cases where the work period states " estimated," prior notification that work is to extend beyond the period indicated is not necessary to completing the assigned work.
PROPOSAL CONTENT The minimum items required for preparation of all DOE proposals (NRC Form 189) are:
1.
Protect Title - The title should be a unique identifier of the project reflecting a general ideal of the encompassed work.
In most cases, the title on the NRC Statement of Work (S0W) should be used.
2.
Date of Proposal - Include revision number if applicable.
3.
NRC Office - Indicate the NRC Office (s) supporting or requesting the work.
4.
FIN Number and B&R Number - Indicate NRC's FIN Number and B&R Number, as provided on the 50W.
5.
DOE Contractor - Performing organization's name.
6.
Site - Indicate location (s) of organization that will perfom the work.
7.
Contractor Account and DOE B&R Number - Highest level feasible.
8.
Cognizant Personnel - Region V Project Manager and other NRC Technical Staff, if applicable; DOE Project Manager; and the performing organization's key personnel: Project Manager and principal investigator (s), and FTS phone number; include their resumes.
9.
Period of Perfomance - Provide proposed date of connencement of work and best-estimated completion date of the project.
- 10. Staff-Years / Months of Effort and Costs - Estimate to be incurred by DOE contractors, subcontractors, and consultants. List by fiscal year from start to completion of the project (FIN). Cost estimate will be consistent with DOE Manual 2200, the Pricing Manual, and shown in thousands of dollars.

- 11. Monthly Forecast Expense - Provide the planned monthly rate of costs by fiscal year. This may be provided with the first report of an authorized program if not known at time of proposal submittal. At the beginning of each subsequent year, reports should include the planned monthly rate of costs for the ensuring year.

Work Requirements - Describe the tasks and subtasks to be performed in such a manner that a disinterested third party can understand what is to be done and how it will be accomplished for the level of effort and period of performance proposed. Describe any assumptions being made pertinent to perfonning the work. Provide criteria or other bases upon which evaluations, judgment or other action will be taken. Describe potential problems and recommend solutions. State and describe separately any options being proposed that require NRC approval before proceeding.

Reporting Requirements - State the number, type, and other content of the reports requested or proposed to be provided, to whom and when they are to be submitted, and, if requested by NRC, whether drafts will be provided prior to finalization.

Meetings and Travel - State the number of trips that the perfonning organization will require to perform the work and identify where and how many people are expected to travel.
If no travel is expected or required, state none.
Foreign travel must be addressed separately and approval must be obtained by processing NRC Form 445 in addition to being provided as part of the approved proposal.
