BI-MONTHLY BAILEY FAILURE REPORT HOPE CREEK GENERATING STATION DOCl:ET NO. 50-354 Pursuant to commitments contained in the Public Service Electric and Gas Company (PSE&G) letter to the NRC dated October 15, 1986, attached please find the Bailey 862 Solid State Logic Module (SSLM) Bi-Monthly Failure Report for the Hope Creek Generating Station (HCGS).
This report covers both lE and non-lE modules and summarizes inservice failures per month by class, assembly type and failure type.
With respect to the last two month period, December 1986 - January 1987, 4 Bailey 862 SSLM failures were identified that had an adverse affect on the circuit in which they were used.
These failures are discussed in the attached Bi-Monthly Report.
Finally, it should be noted that there were no Bailey 862 SSLM failures for the month of December.

L HOPE CREEK GENERATING STATION BAILEY SSLM FAILURES POR DECEMBER 1986 AND JANUARY 1987 4 Modules Failures - There were 4 SSLM failures which occurred in the two month-period beginning December 1, 1986 through January 31, 1987.
Projected Reliability 1.06% per year failure rate
- 1) Based upon data from December 1986 and January 1987, with 0 and 4 f ailures respectively, for a total population of 2258 modules.
- 2) The projected annual failure rate is based upon bi-monthly data to maintain consistency with reports to NRC (Reference letter from C.

Surveillance Activities of the 554 Class lE Tech. Spec. related modules, 116 were exercised through monthly surveillances and operated successfully.
