Action. The requirements contained in enclosure (1) to this instruction are effective upon receipt add will be adhered to by all SIMA Pearl Harbor industrial radiographers.

The management of SIMA, P.H.

establishes anc 1mpienents this QA rogram.

Training'for all QA functicns, prior to ensasement in-tnese functions, is required according to written procedures.

QA Frogram revisions will be made according to written proceoures with ma na g eme n t.a p p r o va l.

The QA Frogram will ensure:that all defined QC proceduresi engineer 1ns procedures, and specific provisions of the package design approval are satisfied.

The QA Program will enphasize control of'the characteristics of the package wnich are critical to safety.

The Raciographic Safety Officer (R50) shall assurs.that alt rac cactive material shippins packages are designed and manufacturec uncer a Quality Assurance.Orogram app. roved by the Nuclear Resulatory Commission for all packases desisned or fabricated after 1 January 1979.

This requirement will be satisfiec ey receiving a certification to this effect from the manufacturer.

All documents related to a specific shippins package will be controlled thrcush the use of written procedures.

All 30cument changes will be performed according to written crocecures approved t.y the Commanding Officer, b.

The Raciographic Safety Officer (R50) shall insure that all QA functions are concucted in accordance iwith the latest soclicable chanses to these documents.

4 Handling, storage and Shipoing a.

Written safety procedures concerning the handlinsi storase, and shipping of packages for certain s.t e c i a l form radioactive caterial wi.11 be followed.

Records of package approvals (including references and crawings), procurement, inspections, tests, operating logs, audit results, personnel training and qualifications and records of shipment will be maintained.

Descriptions of equipment and

Audits a.

Established schedules of audits of the QA.rogram will e

be performed using written check. lists.

Results of-audits will be maintained and reported to management.

Audit reports will be

.evaluatec and deficient areas corrected.

The frequency of the audits will be dependent on the safety significance of the activity being auditedi but shall-be audited at-least once per year.

Audit reports will,be maintained as'part of the quality assurance records.

Members of the audit team shall have~no responsibility in the activity being audited.
