Dear Mr. Gagliardo:

This letter is to document the telepaone conversation between Mr. Craun, Mr. Holmes and Mr.

Jaudon onducted on June 20, 1986 concerning PSC's response to the Notice of Violation 86-08B.

Paragraph 8, " Failure to Control Design Changes", contains an error and the revised statement is hereby submitted.

As stated in PSC's Letter of Response dated June 17, 1986, the following statement was made in error:

"...Since the CN/CN reissue required box was incorrectly checked "No",

subsequent reviews which occur during the "As Built Verification" process would not have found that a CN/CN reissue was required.

P-86437 June 24, 1986 Since the "As Built Verification" (ABV) process at Fort St. Vrain is a field walkdown and verification that the field matches-the design documentation, the ABV process would have found and corrected the deficiency.

Consequently, the above referenced statement should be revised as follows:

"Even though the CN/CN reissue required box was incorrectly checked "No" on the CN-1902 CWP/DR and CN-1798A was referenced to perform the document update, subsequent reviews which occur during the "As Built Verification" process (not yet completed at the time of issuance of the violation) would have found that a CN/CN reissue was required."
