' NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee:.H&G Inspection Company, Inc., Houston, Texas General License (10 CFR Part 50)

Subject:

PROPOSED IMPOSITION OF CIVIL PENALTY - $2,500 f

This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of Two Thousand Five Hundred Dollars

($2,500) will be issued on or about July 21, 1986~to H&G Inspection Company.

This action is based on several violations that collectively represent a lack of effective management control and oversight of licensed activities. The violations in-cluded failure to perform surveys after each radiographic exposure and failure to follow operating procedures. As a result of these failures, a radiographer's assistant received a whole body dose of 3.4 rems in the fourth quarter of 1985. The violations demonstrate the need for significant improvement in H&G Inspection Company's administration and control over the radiation safety program to assure adherence to NRC requirements.

It should be noted that the licensee has not been specifically informed of the enforcement action. The Regional Administrator has been authorized by the Director,' Office of Inspection and Enforcement, to sign this action. The schedule of issuance and notification is:

Mailing of Notice July 21, 1986 Telephone Notification of Licensee July 21, 1986 A news release has been prepared and will be issued after the licensee receives the Notice. The State of Texas will be notified.

The licensee has thirty days from the date of the Notice in which to respond.

Following NRC evaluation of the response, the civil penalty may be remitted, mitigated, or imposed by Order.
