in accordance with Section G.4(c) of the subject contract, entitled " Task Order Award," this letter definitizes the subject task order. This effort shall be performed in accordarice with the enclosed Statement of Work.

Task Order _No. 7 shall be in effect from April 27,1999 through April 30,2000 with a cost ceiling of $18,436.97. The amount of $17,230.81 represents the total estimated reimbursable costs, the amount of $1,206.16 represents the fixed fee.

The obligated amount of this task order is $17,904. This amount shall not be exceeded until notice is provided to you that additional funds are available. It is estimated that this obligated amount will cover performance of work through March 2000.

Over the past four years, the NRC Human Factors Assessment Branch has implemented the Human j

Factor Information System. In order to keep the HFIS data up to date, the LERs from March 6,1999 to April 30,2000 must be reviewed and coded.

Obiective j

The objective of this task order is to obtain assistance for the review and coding of LERs beginning in April 27,1999 through April 30,2000. LERs need to be reviewed using the HFIS Coding instructions, and that information must be incorporated into HFIS. Additionally, monthly and quarterly analysis will be required as identified in the deliverables.

Technical and other Special Qualifications Reauired One nuclear specialist, with expertise in evaluating human performance, for review and coding of human performance issues in LERs.

It is the responsibility of the contractor to assign technical staff, employees, or specialists who have the required educational background, experience, or combination thereof to meet both the technical and regulatory objective of the work specified in this statement of work. The NRC will rely on representations made bJ,the contractor conceming the qualification of the personnel assigned to this task order including assurance that all information contained in the technical and cost proposals, including resumes, is accurate and truthful.2

Deliverables

1. Human performance related information from LERs reviewed using the HFIS coding instructions ready to be entered into the HFIS database at NRC Headquarters located in Rockville, MD on a monthly basis.

2.. Monthly reports showing analysis of LER data entered that month compared with total LER data.

Monthly reports must provide data on the following: - general areas of contributing causes such as training and procedures - personnel categories - type of work categori.es

3. Quarterly reports showing analysis of LER data entered in the previous quarter compared with LERs for the previous 12 month period. Quarterly reports will provide data on the same categories reported in the monthly report.
4. Quarterly reports showing analysis of LER data for the current quarter compared with data from the previous quarter. The quarterly report should provide detailed analysis of any general area pf contributing cause when current quarter results vary more than 10 percent deviation from the previous quarter.
