ENCLOSURE 1 NOTICE OF VIOLATION

- License Nos. DPR-77, 79 During the Nuclear Regulatory Commission (NRC) inspection conducted on June 6-10, 1988, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement a

Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Technical Specification 6.11 requires that procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure.

Licensee Procedure HPSIL-5, Airborne Radioactivity Surveys, Revision 30, April 1,1988, requires that air samplers be placed as close as possible to the breathing zone of a worker and that this distance should be no greater than one or two feet. Air samplers should be kept off floors.

Contrary to the above, the licensee failed to either establish or to adhere to procedures for personnel radiation protection in that:

On June 6,1988, the licensee did not properly perform breathing zone air sampling for a worker in the Unit 2 containment purge filter bank room in that the sampler was placed on the floor at the entrance to the area and at a distance greater than six feet from the worker.

The licensee had no procedure for beta radiation exposure control evaluations prior to steam generator work or for computing, adminis-tering, and documenting work area stay times.

Informtl section guidelines were used in lieu of approved procedures.

This is a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Wasnington, DC 20555, with a copy to the Regional Administrator, Region II, a copy to the Director, Of fice of Special Projects, and a copy to the NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transr.itting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
