Inquiry No. 86-02 This refers to a telephone inquiry by Mr. Steven Courtenmanche of this office with yourself on December 12, 1986. This inquiry concerned activities auth-orized by License No. SNM-1524, as they relate to the activities being conducted dt 3356 Commercial Avenue, Northbrcok, Illinois.
From this discussion, it is our understanding that operations have ceased at this location and that you plan to update your license by submitting new descriptions of places of use to this office with a request to amend your license to include them.
If our understandings are incorrect, please inform us in writing.
We will review this matter during the next inspection of your licensed program.
