APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket No.: 50-458 River Bend Station License No.: NPF-40 During an NRC inspection conducted on February 3-7 and March 3-7, 1986, a violation of NRC requirements was identified. The violation involved failure to have procedures to incorporate required documents in quality data packages.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed l

below:

Failure To Have Certain Maintenance Procedures River Bend Station Technical Specification 6.8.1.a requires that written procedures shall be written, implemented, and maintained covering the l applicable procedures recommended in Regulatory Guide 1.33, Revision 2, Appendix A. Section 9 of Appendix A, requires procedures for performing maintenance.

Contrary to the above, the licensee failed to have: 1) administrative l procedures requiring the incorporation of Vendor Technical Information (VTI) i in maintenance procedures and the use of the latest available revision of l VTI; 2) procedures properly prescribing preventive maintenance intervals; and 3) procedures implementing root cause analysis of equipment failures as defined in ANSI Specification N18.7-1976, Section 5.2.7.1.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Uti.lities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, i including for each violation: (1) the reason for the violations if admitted, l (2)thecorrectivestepswhichhavebeentakenandtheresultsachieved, '

(3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
