DAVIS-BESSE INSPECTION REPORT 50-346/99001(CRP)
Deat Mr. Campbell:
On February 12,1999, the NRC completed an inspection at the Davis-Besse site. The
enclosed report presents the results of that inspection.
Overall, plant activities were characterized by conservative decision making and execution.
For example, engineering personnel aggressively pursued the source of a smal. component
cooling water system leak and plant management conservatively decided to isolate letdown
system cooler 1-1 when it was determined that one of its rupture discs was the source of the
leak. However, emergency diesel generator 2 was inadvertently rendered inoperable due to
inattention-to-detail by operators during the generation and review of a tagout and a lack of a
questioning at2rJe by operators while hanging the tagout.
