Response to Inspection Report STN 50-482/86-05
Dear Mr. Johnson:
his letter is written in response to your letter of May 7,
- 1986, which transmitted Notice of Violation 50-482/8605-01. As requested, the violation identified in the Notice of Violation is being addressed in four parts.
(a)
We reason for the violation if admitted; (b) te corrective steps which have been taken and the results achieved; (c)
The corrective steps which will be taken to avoid further violations; and (d)
The date when full ecapliance will be achieved.
h Violation (482/8605-01): Failure to Document the Performance of a
Surveillance Test in Accordance with the Procedure Finding:
%chnical Specification 6.8.1 requires that,
" Written procedures shall be established, inplanented,... covering... a.
The applicable procedures recomnended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978."
Surveillance Procedure STS GK-001B, Revision 5,
" Control Room Dnergency Ventilation System Train B Operability Test," has been established and inplemented in accordance with the above Technical Specification.

Mr. E. H. Johnson RMLNRC 86-105 Page 2 June 5, 1986 Contrary to the above, on March 20, 1986, the NRC inspector observed STS GK-091B, Revision 5, being performed without Section 3.0, " Initial Conditions,"
first being signed as conpleted.
Reason For Violation If Adnitted:
'Ihe individual who initiated surveillance procedure STS GK-901B, Revision 5, consciously verified the initial conditions required by section 3.8 of the surveillance prior to performing the test but failed to sign them as couplete before continuing into the actual test.
Corrective Steps Which Have Been Taken and Results Achieved:

" Initial Conditions," was signed as conplete whan the failure to sign the steps was identified.
'Ihe incident was discussed with the individual who initiated the procedure by the Superintendent of Operations to stress the inportance of properly docmenting the coupletion of procedural steps requiring signoff and also with the relieving shift personnel to stress the need to perform a review of surveillance tests in progress which are continuing into the next shift.
Corrective Steps Which Will Be Taken To Avoid Further Violations:
A copy of the violation and this response will be placed in Operations Required Reading to ensure operations personnel are aware of the inportance of properly docmenting the coupleticn of procedural steps at the time of performance.
