WORK-AT-HOME REQUEST OR BEVERLY J. SWEENEY This is a request for work-at-home to accommodate Beverty J. Sweeney, Information Management Assistant, Generic issues and Regulatory improvements Section, Generic issues, Environmental, Financial and Rulemaking Branch, Division of Regulatory improvement Programs, Office of Nuclear Reactor Regulation. Ms. Sweeney will be the primary care giver for her newbom child and requests to work-at-home for a period of three months. A work-at-home plan and her medical approval is attached.

. . ATTA0KNT 1 WORK-AT-HOME PLAN

1. Basis of the Work-At-Home Plan To allow Beveriy Sweeney to work at home due to a personal hardship which is caring for her newbom child.
2. Timing Work at-home will begin as soon as Ms. Sweeney ends her matemity leave and feels she is able to work. Due to the fact that Ms. Sweeney will be caring for a newbom, we are proposing that the work-at-home period be limited to not more than 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> per working day for a total of 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> per week. Ms. Sweeney plans to work at home for a period of three months unless an extension of this time is required. Request for extensions will be made in writing following the same process as the initial request to participate in the program. Towards the end of her work-at-home program, if she feels it is necessary, Ms. Sweeney plans to come to work on a part time basis. All absences for Ms. Sweeney will be covered by sick leave or annual leave.
3. Travel Travel will not be required during the work-at-home period.
4. Work Monitorina Ms. Sweeney will be responsible for reporting to her supervisor, Frank Akstulewicz. She ,

will report to him on a weekly basis with the following information: )

1. Work she performed at home.
2. Work hours (needed for time cards).
3. RITS information. 1 Occasionally, Ms. Sweeney plans to visit the office in order to meet with her supervisor and to transfer work to the office.
5. Overtime Overtime will not be authorized during the period of this work program.
6. Workina Hours Working hours will be flexible in order to accommodate Ms. Sweeney. Any telephone contact required will be done during normal NRC working hours. During this work program, the scheduling of the hours will be at Ms. Sweeney's discretion, but is limited to no more than 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> per week.

7. Reports l

l j Ms. Sweeney will keep a daily record of her work hours and activities and will be l- ' responsible for submitting them to her supervisor on a weekly basis. She will also certify  ;

l her time and attendance inputs by telephone, if necessary.

8. Termination of the Work-At-Home Proaram Ms. Sweeney understands that her supervisor may terminate this program if he feels that work activities and progress cannot be adequately monitored.  !
9. Tasks to be Performed at Home Ms. Sweeney will continue to perform her duties as information Management Assistant.

She will continue to serve as list owner for generic communications. She will also  ;

continue to format generic communications in HTML format as well as placing them on '

the intemal server and having them placed on the extemal server. She will continue to l

respond to any requests from contractors and subscribers. If applicable, she will be I responsible for compiling the intemal and public versions of the July and October -

Director's Quarteriy Status Report, and will modify these reports on the intemal server.

She will continue to update DRIP branch homepages and will make any necessary amendment changes to the branch Environmental Protection Plan document she 4 created on the web. She will also format the existing TXT documents on the  ;

Environmental Protection Plan document and change them to HTML documents. If I applicable, she plans on creating databases or complicated drawings / slides for RGEB members. Ms. Sweeney will continue to maintain all her work responsibilities, if applicable, as well as any other duties assigned to her by her supervisor.

10. Soecial Eauloment Reauired Ms. Sweeney will need a computer, a modem, and a printer (if applicable) installed in her home. This computer should have the appropriate software installed in order to perform her duties at home. She will also need access to the LAN.
11. Final Evaluation of Work-At-Home Prooram At the termination of this program, Ms. Sweeney and her supervisor, Frank Akstulewicz l

will prepare written evaluations of this work at-home program for the Office of Human '

Resources.
