NRC INSPECTION REPORT 50-334/98-09,' 50-412/98-09 AND NOTICE OF

VIOLATION

. Dear Mr. Cross:

This refers to the Safety System Engineering inspection conducted from November 16 to

.

December 17,1998, at the Beaver Valley Power Station. The inspection focused on the Unit 2

High Head Safety injection System in its normal and emergency configurations. The team also

. inspected the effectiveness of the engineering organization in ident;fying, resolving, and

]

preventing problems. The enclosed report presents the results of that inspection. The results

were also discussed via teleptione with you, Mr. S. Jain, and other members of your staff, on

December 17,1998.~ Subsequent to that meeting, on January 4 - 6,1999, we conducted

additional in-office inspection. The results of the additional inspection were discussed, via

telephone, with Mr. B. Sepelak of your staff, on February 4,1999.

Based on the results of this lnspection, the team determined that the engineering organization

provides acceptable support to plant operations. In the past, Engineering failed to thoroughly

evaluate and promptly resolve equipment issues. However, the steps you have taken, such as -

increased oversight and root cause analysis technique training, are addressing the weakness.

The team also determined that one violation of NRC requirements occurred. This violation,

pertaining to inadequate design control in the Unit 2 de voltage drop calculation, is cited in the

enclosed Notice of Violation and the circumstances surrounding it are described in detail in the

subject inspection report. The calculational errors were significant in that they could have

resulted in some circuit breakers not having sufficient voltage to close and rendered the

associated accident mitigating components inoperable. Opportunities were available to identify

the errors during the normal review of the calculation as well as during your safety system

functionalinspection of the 480V and 4160 V systems.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

' determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
