ORDER MODIFYING THE LICENSE (IMMEDIATELY EFFECTIVE) AND PROPOSED IMPOSITION OF CIVIL PENALTIES - $300,000 This is to inform the Commissicn that an Order Modifying the l.icense (Effective Immediately) and a Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $300,000 will be issued on or about July 3,1986 to Detroit Edison Company for its Fermi-2 facility. This action is based on several violations that represent a serious breakdown of management controls and discipline in the control room during a reactor startup on July 1-2, 1985. The violations involve the error by a reactor operator in pulling eleven control rods out of sequence, the lack of oversight by supervisory personnel of control room activities during a significant plant evolution, and the failure to evaluate the significance of the control rod pull error and take appropriate corrective actions before resuming startup. The Commission approved issuance of this action on July 3,1986.

It should be noted that the licensee has not been specifically informed of the enforcement action. The Regionai Administrator has been authorized by the Director, Office of Inspection and Enforcement, to sign this action. The schedule of issuance and notification is:

Mailing of Notice July 3, 1986 Telephone Notification of Licensee July 3, 1986 A news release has been prepared and will be issued after the licensee receives the Notice. The State of Michigan will be notified.

The licensee has thirty days from the date of the Notice in which to respond.  ;

Following NRC evaluation of the response, the civil penalty may be remitted, '

mitigated, or imposed by Order.
