SUBJECT:
Special Inspection No.94-005 - q On April 19,1994, Steve Shaffer and Richard inspection at your facility of activities authorized under an NRC general license.
Lulun of th ial safety' inspection was an examination of your licensed activities as they relate to radiation safety and to compliance with the Commission's regulations and the license conditions. The inspection consisted of observations by the inspector, interviews with personnel, and a selective examination of wpiwtative records. 'Ihe findings of the inspection were discussed with you at the conclusion of the inspection.
The inspection resulted in the identification of three apparent violations. The violations involved (1) the unauthorized removal of your Kay Ray Model 7063P density gauge, (2) the failure to ensure that the gauge was adequately secured once it was removed, and (3) the failure to properly post the area where the gauge was being stored. The Kay Ray gauge contained 500 millicuries of cesium-137. We are particularly concerned with your unfamiliarity with the regulatory requirements associated with possession and use of the gauge.
i During the inspection your staff secured the gauge in a locker. It is our understanding that !
the gauge will be secured until you dispose of it and it will be posted with a radioactive l materials caution sign. Also, we understand your doal is to dispose of it by the end of the month. If your intentions are different then those stated above, please call me at 610-337- i 5258. In your response to this letter, please include a confirmation that the gauge has been permanently disposed of or describe the status of your disposal efforts.

Please use the enclosed self-addressed green envelope when you submit your copy to the Regional Administrator, Region I. This will assist us in the timely processing of your response. In accordance with Section 2.790 of NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and your reply will be placed in the Public Document Room. subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
