LICENSE CONDITIONS

10. Radioactive material shall be used only at the following locations:

(a) 2424 Gundry Avenue, Signal Hill, CA (limited to storage and shielded room facilities only).

(b) 2420 Gundry Avenue, Signal Hill, CA (limited to storage of Cobalt 60 source in a trailer).

(c)

Le-yjob sites of the licensee in areas not under exclusive (See Cardtion 29) fedem! jurisdiction throughout the State of Califomia.

11. This license is subject to an annual fee for sources of radioactive material authorized to be possessed at any one time as specified in items 6,7, 8 and 9 of this liceme. The annual fee for this license is required by and computed in auudmw with Title 17, Califomia Code of Regulations, Sections 30230-30232 and is also subject to an annual cost-of-living adjustment pursuant to Section 100425 of the Califomia Health and Safety Code.

16. Quantitative analytical assays for the purpose of tests for leakage and/or contamination of sealed sources shall be performed only by persons specifically authorized to perform that service.
17. The following individuals are authorized to collect wipe test samples of sealed sources pn-wd under this bcense using leak test kits MW-bic to the Califamia Department of Health Services:

(a) The Radiation Safety Officer (b) Qualified individuals designated in writing by the Radiation Safety Officer

18. Records ofleak test results shall be kept in units of microcuries and maintained for inspection. Records may l-be di=v=d of following Department inspection. Any leak test revealing the presence of 0.005 microcuries l

or more ofremovable radioactive material shall be reported to the Department ofHealth Services, Radiologic Health Branch, 601 N. 7th Street P.O. Box 942732, Sacramento, CA 94234-7320, within fik days of the test.

1his report shall include a description of the defective source or device, the results of the test, and the l

conective action taken.

19. The licensee shall conduct a physical inventory every six months to account for all sealed sources and/or devices received and ;-= +d under the license. Records of the inventories shall be maint,ined for inspection, and may be diW of following Department inspection.
20. The licensee shall comply with all requirements ofTitle 17, Califamia Code of Regulations, Section 30373 l

l when L..gnting or delivering radioactive materials to a carrier for shipment. These requirements include; (n=+=ing, marking, labeling, loadmg, storage, pis ding, monitoring, and accident reporting). Shipping papers shall be maintained for inspection pursuant to the U.S. Department ofTiiwpuriation reqtaumu (Title 49, Code of Federal Regulations, Part 172, Sections 172.200 through 172.204).

21. Pursuant to Title 17, Califomia Code ofRegulations, the licensee is authonzed to possess up to 999 kilograms (2,203 pounds) of natural or depleted tranium used for purposes of shielding or collimation in radiographic exposure devices listed in Item 9 of this license.
22. At any time the licensee is en==d in the performance ofindustrial radiography by authority of this license, at either a permanent or a taupci-yjob site, he shall have a current copy of each of the following documents available for inspection at the site:

(a) Title 17, Califamia Code of Regulations.

(b) This license.

(c) The licensee's operating and emergency procedures.

23. The licensee shall not authorize any radiographer or radiographer's assistant to use any model of a radiographic c:xposure device, related handling tool, or radiation survey instrument until the radiographer or radiographer's assistant has been trained in the use ofsuch equipment, and has &irumbated competence in its use. For each indiographer or radiographer's assistant, the licensee shall mamtain the following records available for inspectiort (a)

Training received by the radiographer or radiographer's assistant in the use ofsuch equipment, including dates, duration, and name of instructor.

1 (b) Each radiographic exposure device for which the radiographer or radiographer's assistant has demonstrated competence in its use, including date of siruronation of competence.

24. (a)

The licensee shall conduct refresher training for all radiogogics and radiographers' assistants at intervals not to exceed six months. Such training shall include, but not limited to:

i (1)

Title 17, Califomia Code of Regulatiois, with emphasis on Sections 30330-30334.

(2)

Subject matter listed in Title 17, Cadfomia Code of Regulations, Section 30335.

(3)

This license.

(4)

The licensee's operating and emergency procedures, with emphasis on any changes which have been made within one year.

(5)

Equipment used under this license for performance ofindustrial radiography with emphasis on equipment which has been used by the licensee for less than one year.

(6)

Prevention of omwgxsares of purunel, with emphasis on prevention of actions and ciretme-e within the past year which have caused, or threatened to cause, 1

25. The licensee shall not use 50 or more curies of Cobalt 60 or more than 100 ctaies of Iriditun 192 for performance ofindustrial radiography at tcupmyjob sites except under the following provisions for each site:

(a)

Prior to perfomance ofindustrial radiography, the Radution Safety Officer or an Altemate Radiation Safety Officer shall physically inspect the area where radiography is to be performed, and shall decide l

on location (s) for the exposure device and source tube positioning.

l (b) Performance ofindustrial radiography shall commence only after written authorization by the Radiation Safety Officer or Attemate Radiation Safety Officer, which shall contain the following items:

(1)

Imation of operatioa (2)

Make, model, and serial nturber of exposure device.

l (3)

Specific time pericd (date, time, shift, etc.).

(4)

Detailed diagram of proposed exposure area, including the following items:

a.

Placement of exposure device, taking consideration for maximum safe access in case of equipment malfunction.

b.

Source tube positioning.

I c.

Collimation.

l l

d.

Additional shielding.

e.

Appuxinate perimeter lines and calculated dose rates.

f.

Calculated time per expostne.

g.

Estimated number of expostnes to be taken.

(c)

The licensee shall maintain available for insp:ction nxxrds of all such activities, including a copy of each written authorization.

26. Sources of radioactive material shall be changed in each radiographic expostne device only in accordance with instructions of the distributor of the source changer. 'Ihe individuals listed below are the only ones authorized by this license to change sources in radicgraphic exposure devices:

(a)

The Radiation Safety Officer.

27. No individual shall act as a radiographer or as a radiographer's assistant taless at all times during radiographic

. operations, each such individual vears a direct readmg pocket dosimeter, an alanning ratemeter, and either a film badge or thermoluminescent dosimeter as required by the California Code of Regulations, title 17, Section 30333.2. Alarming ratemeters are not required for pmweit radiographic facilities where other oppuriate alarming or wammg devices are in routme use.

28. Notwitioi ding the authorized uses of radiographic ~;nipmmt under Item 9 of this license, the use of any mdiographic sealed source, exposure device, sotree changer, or aWa'M equipment which does not meet the requirements of the Califamia Code of Regdatinm-title 17, section 30332, is prohibited except for purposes of storage, transfer, and disposal.
29. Before mdioactive materials may be med at a temporary job site at any federal facility, the jurisdictional status of the job site east be deternined. If the juisdctional status is udmown, the federal agency should be contacted to determine if the job site is iederexclusive federal janisdction. A respome shall be ohnised in writing or a record made of the pane and tide of the person at the Federal agency who provided the determinmaion and the date that it was pmvided. Authorization foruse of radoactive materials at the job sites under exclusive Federal jurisdction shall be ohninal either by:

(a)

13. Execpt as specifically provided otherwise by this license, the licensee shall possess and use radioactive material described in items 6,7,8 and 9 of this license m acccrdance with the statements, representations, and procedures contained in the documents listed below.

The Department's regulations shall govern unless the statements, representations, and procedures in the licensee's application and correspondence are more restrictive than the regulations.

(a) The Radiation Safety OtYicer.

(b) Qualified radiographers designated in writing by the Radiation Safety Officer.

24. No individual shall act as a radiographer or as a radiographer's assistant unless at all times during radiographic operations, each such individual wears a direct reading pocket dosimetsr, and alarming ratemeter, and either a film badge or thermelumincscent dosimeter as required for permanent radiographic facilities where other appropriate or warning devices are in routine use.
25. The licensee shall comply with all requirements of Title 17, California Code of Regulations, Section 30373 when transporting or delivering radioactive materiala to a carrier for shipment. These requirements include; packaging, marking, labeling, loading, storege, placarding, mor.itoring, and accident reporting. Shipping papers shall be maintained for inspection pursuant to the U.S. Department of Transportation requirements (Title 49 Code of Federal Regulations, Part 172, Sections 172.200 through 172.204).

26. Before radioactive materials may be used at a temporaryjob site at any federal facility, the jurisdictional status of the l

job site must be determined. If the jurisdictional status is unknown, the federal agency should be contacted to determine if the job site is under exclusive federaljurisdiction. A respona shall be obtained in writing or a record made of the name and title of the person at the federal agency who provided the determination and the date that it was provided. Authorization for use of radioactisc materials at thejob sites under exclusisc federaljurisdiction shall be obtained either by:

(a) Filing an NRC Form-241 in accordance with the Code of Federal Regulations, Title 10, Part 150.20 (b),
