LASALLE INSPECTION REPORT 50-373/99003(DRP); 50-374/99003(DRP)

Dear Mr. Kingsley:

This refers to the inspection conducted April 1 through May 12,1999, at the LaSalle County

Station. The purpose of the inspection was to determine whether activities authorized by the

license were conducted safely and in accordance with NRC requirements. At the conclusion of

the inspection, the findings were discussed with those members of your staff identified in the

enclosed report.

Overall, Unit 1 continued operating safely while considerable station efforts and resources were

devoted to supporting the restart of Unit 2. The Unit 2 restart activities were conducted safely

and plant personnel and equipment generally performed well. However, the NRC noted several

examples of weaknesses in the areas of operator log keeping, corrective action program

implementation, and procedures. The examples noted did not result in events with safety

significant consequences; however, they collectively represented weakened barriers that you

established to ensure sustained, safe operations. The NRC noted improvements and

corrections as the startup activities progressed. You are encouraged to continue your focus in

these areas.

Based on the results of this inspection, the NRC ha determined that two violations of NRC

requirements occurred. The first violation involved two examples of failing to follow procedures.

In one case, operators failed to log suppression pool temperatures as required during reactor

core isolation cooling system testing. In the second example, operators failed to ensure proper

valve configuration associated with the turbine stop valve / turbine control valve fast closure

alarm. The second violation involved an inadequate procedure: an annunciator response

procedure in the main control room associated with Unit 2 drywell oxygen concentration levels

was incorrect and did not apply to the designated alarm. These violations are being treated as

Non-Cited Violations (NCVs), consistent with Appendix C of the Enforcement Policy. The NCVs

are described in the subject inspection report. If you contest the violation or severity level of

these NCVs, you should provide a response within 30 days of the date of this inspection report,

with the basis for your denial, to the Nuclear Regulatory Commission, ATTN: Document Control

Desk, Washington DC 20555-0001, with copies to the Regional Administrator, Region lil, and

the Director, Office of Enforcement, United States Nuclear Regulatory Commission,
