Dear Mr. Spence:

We have received your response (Reference 1) to my letter of August 1, 1986 and find it necessary to provide some clarification to the contents and schedule for the Phase 4 report.

Regarding the schedule, you state that TUGCO anticipates issuance of the Phase 4 report within tour weeks of our receipt of responses to the open items letters. Cygna's understanding of the scheduling commitments, however, was that the Phase 4 report would be issued within four to six weeks of our determination that satisfactory responses have been received. Included 1n this six week period are two weeks for Senior Review Team comments and their resolution. Relative to the open items letters, last week we received responses to approximately half of our open items (References 2 and 3) and have begun our review of these responses. We will provide a status of all open items to date to you in the near future Y

i Mr. M. D. Spence 84056.104 Page Two However, in addition to the open items outlined above, there are also unresolved issues that pertain to our questions on the CPRT plan. At the request of TUGCO management, our questions / comments on the Plan were issued in letters 84056.85 and 84056.091 dated October 16 and October 21, 1985, respectively. We agreed with you on October 1, 1985 and May 30, 1986 that they would be resolved in meetings with Stone & Webster and/or CPRT. No meeting schedule has been established for resolution of these items. Although we are not making any predetermination as to the completeness of the responses received to date, it does not appear that Cygna's questions on the Plan have been fully addressed. In the area of piping and pipe supports, we received and are reviewing the Stone & Webster procedures. Some clarification as to how the Cygna issues have been addressed appears to be required. We have not received any procedures or further details in the cable tray / conduit support, mechanical systems, electrical, I&C, or design control disciplines.

Relative to the contents of the Phase 4 report, the Reference I response indicates that other than closing individual open items, TUGCO does not want Cygna to address all of the review issues documented in the RIL. Specifically, in the area of design control, we will not be able to close out any items relative to the effectiveness of select procedural controls without reviewing the collective significance of the technical findings and their root causes. In addition, the Independent Assessment Program methodology used to date requires that individually significant findings be evaluated in view of other findings to determine whether they are symptomatic of a larger control problem. Please note that in Cygna letter 84056.050 dated January 25, 1985, we indicated the need to consider the results of all four phases collectively prior to documenting any conclusions in the Phase 4 report. This letter essentially served to withdraw the I

conclusions provided in previous phases pending the cumulative ,

effects evaluation to be completed as part of the Phase 4 effort.
