RNCLOSURE 2 BULLETIN i

The Employee Concerns Special Program (ECSP) has established a Report Review Station at Sequoyah Nuclear Plant (SQN). The purpose of this station is to provide the opportunity for all~ employees, including those who voiced concerns about Sequoyah Nuclear Plant, to review reports which describe TVA management actions to resolve the concerns of employees.

The evaluation methodology, the conclusions of the evaluation teams, and l'

corrective actions, where applicable, are described in reports which were written by members of the Employee Concerns Task Group (ECTG). The ECTG is comprised of contractors, TVA managers, and salary policy personnel.

This station is located north of the new Power Stores Building at SQN in the trailer complex near the warehouses. There will be signs on site directing personnel to the review station.

The station will be in operation from to Operating hours will be from 0600-1800 Monday through Friday. However, these dates and times will be expanded as i

required. - Telephone numbers will be extension 7407 and 7420. Also toll free numbers have been provided for convenience. The numbers are: 1-800-345-4096 in Tennessee and 1-800-345-0680 for out-of-state calls.

A staff will be provided to assist in locating reports and to answer questions about the program. Cubicles and reference materials will be 3

available for report review.

February 1,1986 was established as a cutoff date for concerns to be included in the WBN ECSP. All concerns received before February 1,1986, are being evaluated by the ECTC.

All concerns received after February 1, 1986 will be evaluated by the ongoing Employee Concern Program (ECP) which was established to handle employee concerns as they arise.

The ECTG divided the concerns into nine major categories for evaluation.

The major categories being evaluated by the ECTG are:

Management and Personnel Relations (M&P)

Industrial Safety (IS)

Construction Material Control Operations Quality Assurance / Quality Control Welding Engineering Intimidation, Harassment and Wrongdoing (I&H)

I&H concerns are being investigated by the Inspector General.

M&P and IS concerns are not restart items for SQN but are being addressed on a corporate level. They will not be available for review at this time.

ENCLOSURE 3 TVA EMPLOYEE CONCERNS SPECIAL PROGRAM REPORT REVIEW STATTON k

TVA's policy is that employees and other interested parties who voiced concerns with TVA's nuclear program will have an opportunity to review the reports that describe TVA management actions to resolve the concerns of employees. The Employee Concerns Special Program (ECSP) Report Review Station has been established at Sequoyah Nuclear Plant (SQN) north of the new Power Stores facility in the trailer complex near the warehouses. This station will help facilitate such a review.

1-Evaluation reports that address the SQN issues raised in the ECSP are contained within the office -and will be available for your review.

Staff personnel and private cubicles will be provided in this trailer to assist you in your review.

In order to find the report which you are interested in reviewing, you need to know a particular concern number, issue / subject, report number, or category where the concern may be addressed. The staff will show you how to find the applicable report.

The ECSP was established to evaluate and resolve employee concerns expressed primarily through Quality Technology Corporation, the Nuclear Safety Review Staff, and various site managers or management programs before February 1, i

1986. For evaluation of concerns expressed after February 1, 1986, the site Employee Concern Program (ECP) has been established.

The ECSP addressed over 5,800 employee concerns.

Each of the concerne vas documented by the receiving organization as a formal, written description of a circumstance or circumstances that an employee thought was unsafe, unjust, inefficient, or inappropriate.,The mission of the ECSP was to thoroughly investigate all issues presented in the concerns and to report the results of those investigations in a form accessible to ONP employees, NRC, and the general public.

The results of these investigations are communicated by four levels of ECSP reports: element, subcategory, category, and final.

1 An element consists of one or more closely related issues. An issue is a l

potential problem identified by ECTG during the evaluation of one or more related concerns.

For efficient handling, similar concerns were grouped into elements early in the program, but issue definitions emerged from the evaluation process itself. Consequently, some elements did include only one issue, but often the ECTG evaluation found more than one issue per element.

l Subcategory reports summarize the evaluation of a number of elements.

However, the subcategory report does more than collect element level evaluations. The subcategory level overview of element findings leads to an integration of information that cannot take place at the element level. This integration of information reveals the extent to which problems overlap and will, therefore, require corrective action for underlying causes not fully apparent at the element level.

i The subcategories are themselves summarized in a series of eight category reports. Each category report reviews the major findings and collective significance of the subcategory reports.

A category definition sheet has been provided to help you determine which category the concern you wish to review is in.

Although evaluation on all concerns expressed in the ECSP have not been completed at this time, those concerns which raise issues affecting the SQN have been evaluated and completed.

Additional information sheets will be available at the Review Station to aid in the understanding of the ECSP and the resultant reports.

Should you feel that your concern has not been properly evaluated, you are encouraged to discuss your concern with your supervisor and subsequently with higher levels of management. Additionally, you have the right to take your concern to the site ECP organization for evaluation or the other avenues available for resolution of concerns.

Before you visit the review station, it is requested that you adhere to the following:

1.

IF YOU DO NOT REMEMBER YOUR CONCERN NUMBER, NRC WILL HAVE A REPRESENYATIVE AT SQN IN THE RESIDENT INSPECTOR'S OFFICE FROM

2.

IN ORDER TO MAINTAIN YOUR CONFIDENTIALITY AGREEMENT WITH QUALITY TECHNOLOGY COMPANY OR NSRS, PLEASE DO NOT TELL THE STAFF THAT YOU EXPRESSED A CONCERN THAT WAS EVALUATED UNDER THIS PROGRAM.

3.

PLEASE USE THE REVIEW AREAS FOR REVIEWING THE EVALUATION REPORTS.

4.

IF YOU FEEL THERE IS AN ERROR IN A PARTICULAR EVALUATION REPORT, PLEASE NOTIFY THE SITE ECP MANAGER.

STEPS TO OBTAIN THE RESPONSE TO YOUR CONCERN If you know your concern number or are interested in a particular concern number:

1.

Obtain printout No. 1 from the secretary.

2.

Locate the concern number in the left-hand column of the report (concern numbers are listed ALPHANUMERICALLY).

3.

The concern is filed under a subcategory report number. This report number is located in the third column of the printout on the same line as the concern number.

4.

Request the subcategory number found in the third column from the secretary and ask that you be directed to a reviewing booth.

If you do not know your concern number and only know the particular concern content:

1.

Obtain category definition sheet from the secretary.

2.

Determine the category of your concern and obtain a copy of the

" Subcategory Report Listings" from the secretary.

3.

Locate the appropriate category section and plant in the Subcategory Report Listing.

4.

Scan the subcategory column for a title related to your concern.

5.

Give the report number from Column No. 1 and the Element Report number from Column No. 3 to the secretary and ask her/him to get you the report and direct you to a reviewing booth.

After you have reviewed the report, return it to the secretary.

If there are additional reports you wish to see, repeat the steps listed above.
