Entrance Interview:

Following an introduction of all participants, the NRC staff indicated that the day's review would consist of a tour of the site, observation of testing, review of records, and exit interview. Mark Day, State of Utah Project Manager, described the status of remedial action construction and activities presently in progress and indicated that he and his resident manager (Mike Roshek) would be available to accompany the NRC staff on the site tour.

The State of Utah personnel conducted the NRC representatives and the other participants on a tour of the site.

Activities observed include the following major work areas:

1.

Placement and compaction of contaminated material (s) in an area along the west side of the pile.

Contaminated material (s) are high in moisture content when placed on the pile.

Materials are first disced to reduce the moisture content and then the placed contaminated materials are compacted by sheepsfoot roller, followed by a second discing and recompaction.

Rain and snow conditions at the Clive site limited the amount of ongoing construction activity during the site visit.

Testing Observations:

There were no tests run during the site visit due to inclement weather.

Records Review:

The staff selectively reviewed testing and daily inspection records and, with the exceptions below, found them to be complete and to verify performance of construction in accordance with applicable specifications.

Resolution of Issues from Previous Site Reviews:

There were no previous issues to be resolved.

JUN 171986 Exit Interview:

Prior to leaving the site, the NRC staff met with DOE, State of Utah and TAC personnel to discuss issues and observations identified during the site review. The items discussed are as follows:

Issue:

A review of the testing records indicate that only 15 sand cone tests have been run to date on approximately 600,000 cubic yards of contaminated materials. According to construction specifications, this volume of material would require approximately 60 sand cone tests to verify placement.

Also, records indicate that since sp' ring activity began,

Issue:

Steve Matern (Radiation Technician, UD0H) stated that the radiation survey, measurement and radiation data records were maintained in Salt Lake City by the UD0H. However, he explained that the State is presently in the process of moving the above mentioned records to the Clive site. Therefore, NRC staff could not review radiation survey records at the Clive i

site.

Observation:

During the site visit, the NRC personnel were informed that the pile may undergo possible alteration. The alteration will consist of a change in pile ridge slope length. The proposed alteration will not change the degree of slope with respect to the pile. The proposed alteration is based on expected increase in material volume during placement on pile.

Observation: The rail route to the disposal site parallels the great Salt Lake. There is some concern that rising lake levels may interrupt and/or impede rail shipments of contaminated materials going to the Clive disposal site.

_ Site Tour:

The State personnel conducted the NRC representatives and the other participatns on a tour of the Vitro site. Activities observed include the following.

1.

Excavation of contaminated materials.

2.

Verification survey of contaminated materials removal.

3.

Health pnysics staff taking field samples to verify cleanup.

4.

Lay-out of mapped grid areas for cleanup verification.

a.

Each grid is 10 square yards or 33 square feet.

b.

Nine (9) spot samples are collected from each grid to verify cleanup within a grid.

5.

Toured waste water treatment plant at site, 6.

Observed laboratory (radiation survey).

Records Review:

Staff reviewed radiation survey records available at the Vitro site. All available records appeared to be complete and verify perforriance.

Duplicate radiation survey samples arc sent to EPA, Las Vegas, Nevada, for correlation and confidence measurements.

Exit Interview:
