APPENDIX _A NOTICE OF VIOLATION Sacramento Municipal Utility District Docket No. 50-312 P. O. Box 15830 License No. DPR-54 As a result of the inspection conducted between June 2 and July 11, 1986, and in accordance with NRC Enforcement Policy, 10 CFR Part 2, Appendix C, the following violation was identified:

NRC Order, 7590-10, dated March 14, 1983, required.in part that: For NUREG 0737 (Clarification of TMI Action Plan Requirements) Item II.E.1.2, the safety grade initiation and flow indication of the auxiliary feedwater system will be installed during the 1983 refueling outage.

NUREG 0737, Item II.E.1.2, further describes that the auxiliary feedwater system flow rate indication must be consistent with satisfying the requirements of 10 CFR 50, Appendix A, General Design Criterion 13.

General Design Criterion 13 states in part: " Instrumentation shall be provided to monitor variables and systems over their anticipated ranges for normal operation, for anticipated operational occurrences, and for accident conditions as appropriate to assure adequate safety..."

Loss of offsite power is an anticipated operational occurrence which will result in prolonged loss of power to instrumentation.which is not provided with a Class IE power supply.

Contrary to the above, the auxiliary feedwater flow indication in the control room is not powered by a Class IE power supply, nor built to Class IE requirements.

This is a Severity Level IV Violation.

Pursuant to the provisions of 10 CFR 2.201, the Sacramento Municipal Utility District is required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including: (1) corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and, (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
