DOCKET 50-266 REPLY TO NOTICE OF VIOLATION 50-266/88020-03 POINT BEACH NUCLEAR PLANT, UNIT 1 By letter dated October 28, 1988, Region III transmitted the report of a routine safety inspection at Point Beach Nuclear Plant.

The letter stated that cortain activitics appeared to be in violation of NRC requirements and enclosed a Notice of Violation identifying the matter.

Pursuant to 10 CFR 2.201, this letter and the enclosure are in respon:9 to the Notice of violation.

Wisconsin Electric agrees that the violation occurring during a performance of ICP 2.1, "Periodic Test Reactor Protection and Safeguards Analog Channels I Through IV," was properly classified as 3cvority Level IV.

Our discussion of the issue and corrective actions for this item are provided in the attached enclosure.

REPLY NOTICE OF VIOLATION DOCKET 50-266 POINT BEACH NUCLEAR PLANT, UNIT 1 Description on September 27, 1988, during the performance of Unit 1 monthly surveillance procedure ICP 2.1, "Periodic Test Reactor Protection and Safeguards Analog Channels I Through IV," it was noted that the Safety Injection (SI) Unblock Bistable Test Switch was in the trip (open) position.

This switch is required to be in the closed position for normal operation.

As noted in the inspection report, this condition was conservative from a safety standpoint.

Regardless, the switch had not been placed back in its closed position after the completion of the last surveillance testing done on September 14 in accordance with the biweekly surveillance procedure ICP 2.2, "Periodic Test Reactor Protection and Safeguards Analog Channels I Through IV."

Stop 2.44 of ICP 2.2 requires that all bistable test switches be returned to their normal positions.

cause The cause of the switch being left in the open position was personnel error.

The procedures (ICP 2.1 and 2.2) have numerous steps included which direct the return of test switches to their normal position after testing of c group of bistables is complete.

C,orrective Action Personnel involved in the procedural violations were counselled on the need to ensure the procedure is properly completed.

To reduce the probability of recurrence, the test procedures will be revised to require indopondent verification of the test switch positions of those switches that do not result in alarms on the main control board when out of their normal position.

This procedure revision will be completed prior to the end of 1988.
