MODIFICATION NO. 3 TO TASK ORDER NO. 3 ENTITLED " NATIONAL i SEALED SOURCE AND DEVICE REGISTRY SYSTEM" UNDER CONTRACT NO. NRC-02-95-001
Dear Ms. Parker:
The purpose of this modification is to: (1) exercise the optional subtask to continue the development of the National Sealed Source and Device Registry System to provide l implementation via Web Access, including interface integrity testing of front end applications, security testing of front end applications, stress testing of front end applications, and internal NRC and external direct user access of NSS&DR System via NRC Homepage. This module shall be developed with the same development, testing, and documentation criteria used to develop previous modules under the basic task order, and in accordance with the contractor's l
technical proposal dated April 6,1999; (2) increase the obligated and ceiling amounts of the task l order by $24,451 from $174,448 to $198,899; (3) under Section 8.2, revise the estimated scheduled dates for completion / delivery of work for Web Access Development; and l (4) incorporate additional completion / delivery dates regarding documentation and front end testing and implementation for Web Access. Accordingly, the task order is hereby modified as follows:
The second paragraph of the definitization letter of the basic task order is deleted in its entirety and the following paragraph is substituted in lieu thereof:
" Task Order No. 3 shall be in effect from October 1,1998 through September 30,1999 with a cost ceiling of $198,899.00. The amount of $184,338.00 represents the total estimated reimbursable costs and the amount of $14,561.00 represents the fixed fee."
The amount currently obligated under this task order is $198,899.00.
Section 8.2 entitled " Specific Scope," the completion / delivery dates for the module, " Web Access Development," are revised to read as follows:

-Final Version of System Design document to include Web Access 9/15/99 Front End Testing and Implementation
-Revised Test Plan and Procedures to include Web Access 5/30/99
-Conclude Testing of Web Access - 8/30/99 .
-Final Version of Test Plants and Results document l to include Web Access 9/15/99" .
