Dear Mr. Sirs:

i This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period of February 1-28, 1987.

The following response to the items contained in the Notice of Violation is hereby submitted:

Failure to follow Admimistrative Procedure 0-15:

Criterion V of Appendix B to 10 CFR Part 50 states, in part,

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or d rawi r.g s. "

The licensee's quality assurance program as documented in FSAR, Appendix B,

paragraph B.5.5.2, states, in part, "All safety-related maintenance, inspection and surveillance activities are performed to procedures authorized under controls imposed by the FSV Administrative Procedure Manual."

Administrative Procedure Q-15, Issue 6,

" Control of Nonconforming items,"

in paragraph 4.1.2b,

states, "For nonconforming items affecting plant operations quality control shall:

concur with the shift supervisor on placement of the hold tag."

P-87139 April 2, 1987 Contrary to all of the above, Hold Tag 87-042 was placed on Electrical Cabinet I-9366 on February 15, 1987, without obtaining concurrence of the shift supervisor.

The equipment involved did effect plant operations.

(1) The reason for the violation:

The procedure requirement is vague in that it does not provide for acknowledgement by the Shift Supervisor nor does it indicate wher the shift supervisor is to be notified of a. nonconforming candition associated with plant equipment.

(2) 15e corrective steps which have been taken and the results achieved:

A procedure change was initiated by.PDR 87-1299 and PORC approved on March 3, 1987, which clarifies the requirement for informing the Shift Supervisor and obtaining his acknowledgement by signature of all NCR's written against plant equipment.

Administrative Procedure Q-15 now states:

For nonconforming items affecting plant equipment operations Quality Control shall:

a)

Obtain the signature of the Shift Supervisor after the NCR has received a number and prior to the NCR being dispositioned.

b)

Concur with the Shift Supervisor on placement of the hold tag. The Shift Supervisor shall initial the hold tag which inaicates he is aware of the hold status of the equipment.

c)

It is the responsibility of the Shift Supervisor to contact Technical Services for an operability review in accordance with the Technical Specifications for the item on hold.

d)

In the case where an operability review determines that a component is allowed to operate, QC shall identify on the hold tag "May operate in accordance with operability review."

(3) The corrective steps which will be taken to avoid further violations.

PDR-87-1299 is a permanent change to Administrative Procedure 0-15.

The continued implementation of PDR-87-1299 will prevent re-occurrence of this situation.
