PILGRIM STATUS REPORT FOR THE PERIOD OCTOBER 22-NOVEMBER 4, 1988 Enclosed is the Pilgrim bi-weekly status report from the NRC Resident Office at Pilgrim. Three resident inspectors monitored activities at the plant during the report period. The licensee completed major surveillance testing during this period, including the load shedding and loss-of-offsite power surveillance tests. The resident and regional inspectors have been closely monitoring licensea activities and independently assessing plant readiness for restart.

The Pilgriu bi-weekly status report is intended to provide NRC management and the public with an overview of plant activities and NRC inspection activities.

Subsequent inspection reports will address many of these topics in more detail.

. l ENCLOSURE PILGRIM STATUS REPORT FOR THE PERIOD OCTOBER 22 - NOVEMBER 4, 1988 1.0 Plant Status As of 8:00 a.m. on November 4,1988, the reactor was in the cold shutdown mode with moderator temperature at at 87.5 degrees Fahrenheit.

2.0 Facility Operations Summary The plant has been shutdown for maintenance and to make program improve-ments since April 12, 1986. The reactor core was completely defueled on Februa ry 13, 1987, to facilitate extensive mLintenance and modification of plant equipment. The licensee completed fuel reload on October 14, 1987.

Re-installation of the reactor vessel internal components and the vessel head was followed by completion of the reactor vessel hydrostatic test.

The primary containment integrated leak rate test was completed during the week of December 21, 1987.

The licensee completed a formal management self-assessment of their readi-ness for restart and submitted this to the NRC for review on May 26, 1988, in the Pilgrim Nuclear Power Station Self-Assessment of Readiness for Restart Report and Volume 2, Revision 2 of the Restart Plan. The NRC completed its review of the licensee's Self-Assessment Report and con-ducted an Integrated Assessment Team Inspection (IATI) during August 8-24, 1988. The purpose of the IATI was to perform an in-depth assessment of the licensee's readiness for restart. The IATI Inspection Report 50-293/

88-21 was issued on September 7, 1988. The team concluded with confidence that Boston Edison Company (BECo) management controls, programs and per-sonnel are generally ready and performing at a level to support safe startup and operation of the facility. The licensee has formally requested permission of the NRC for restart of the Pilgrim plant.

The licensee has completed major surveillance testing and is continuing with system turnovers in preparation for restart.

3.0 Items of Special Interest Proposed Civil Penalty On November 1,1988, the NRC issued a Notice of Violation and proposed imposition of a civil penalty in the amount of $50,000 for alleged violations at the Pilgrim plant of NRC requirements in 1987 relating to the physical security of the plant and including an alleged material false statement by a former security officer at the facility who had been employed by a Boston Edison Company contractor.

Enclosure 2 ThisenforcementactionstemmedfromtwoNRCphysicalsecurity' TAT $ections at Pilgrim in 1987 and a related inquiry recently completed by the NRC Office of Investigation. The inspection findings identified three instances of degraded physical barriers to areas of the plant containing vital equipment. In addition, the NRC alleged that one of the security lieutenants, when initially questioned by an NRC inspector, made a delib-erate material false statement to the inspector. Details are contained in Inspection Reports 50-293/87-30 and 50-293/87-50.

The technical issues were resolved and the guard lieutenant fired shortly after identification of the violations. Recent inspections have shown marked improvement in the security program during 1908.

Plant Physical Readiness for Restart As of November 4, 1988, all previous NRC restart open items were resolved.

Plant physical readiness is pending review of pipe support issues. On October 14, 1938, an NRC inspector identified two loose base plate anchor bolts on a containment penetration pipe support. Subsequently, the licen-see has been performing extensive inspections and analyses to determine whether a significant problem exists. Licensee conducted walkdowns cf containment penetration pipe supports similar to that in which the loose anchor bolts were initially found and has been analyzing any noted dis-crepancies between the "as-built" configuration of the supports and the design drawings. The licensee has expanded the walkdowns to additional piping supports, component supports and structural supports considered important to safety and 5as been reperforming stress analyses where neces-sary. Two additional loose bolts were found on November 2, 1988, and will be repaired. The NRC staff has been closely following licensee activities and has been independently reviewing the licensee's analyses.

m Enclosure 3 Two region-based specialist inspectors were on-site the week of October 24, 1988, and one region-based specialist inspector the week of October 31, 1988, to review the licensee's actions with respect to the containment penetration pipe support hanger discrepancies. The results of 4 this inspection will be documented in a subsequent inspection report.

Three region-based reactor engineers and the senior resident inspector from the Susquehanna facility were on-site during the week of October 24, 1988, to assist the resident inspectors. The results of these inspection activities will be documented in NRC Inspection Report 50-293/88-33. On October 27, 1988, the Chief, Reactor Projects Section No. 3B, responsible for Pilgrim, the resident inspectors from Millstone Unit I and Vermont Yankee and one additional region-based reactor engineer were on-site to attend plant-specific training in preparation for observing plant restart.
