. cs NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-423 Millstone, Unit 3 License No. NPF-49 EA 87-36 During an NRC inspection conducted between December 29, 1986 and January 7, 1987, NRC inspectors reviewed the circumstances associated with a violation of a technical specification limiting condition for operation (LCO) which was identified by the licensee and reported to the NRC. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1986), the violation is set forth below:

Technical- Specificatic' Limiting Condition for Operation (LCO) 3.5.2 requires that whenever the reactor is in the power operation, startup or het standby condition, two independent Emergency Core Cooling System (ECCS) subsystems shall be operable with each subsystem comprised of several components, including an operable safety injection pump. With 4

one ECCS subsystem inoperable, Technical Specification LC0 Action Statement 3.5.2.a requires that the inoperable subsystem be restored to an operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, or the reactor shall be placed in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

Contrary to the above, from some time during the day shift on November 26, 1986 until 1:00 a.m. on November 30, 1986, a period in excess of 80 hours9.259259e-4 days <br />0.0222 hours <br />1.322751e-4 weeks <br />3.044e-5 months <br /> during which the reactor was at 95% power, the safety injection pump associated with the "B" ECCS subsystem was inoperable and neither was the pump restored to an operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, nor was the reactor placed in at least hot standby within the next six hours. The "B" safety injection pump was inoperable in that its pump cooler service water outlet valve (No. 3SWP*V56) was closed, thereby preventing flow of service water to the pump cooler which is used to provide cooling for the pump lubri-cating oil.

This is a Severity Level IV Violation. (Supplement I)

Pursuant to the provisions of 10 CFR 2.201. "ortheast Nuclear Energy Company is hereby required to submit to this off- vithin thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reason for the violation, if admitted; (3) the corrective steps which have been taken and the resu'ts achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response tima.
