QUALITY ASSURANCE PROGRAM FOR USE AND HANDLING OF R ADIOGRAPHIC SEALED SOURCES Ref:
(a) Title 10 Code of Federal Regulation.
Part 71, Subsection H Encl:
(1) Quality Asairan Program Chart.
- 1. Purpose. To establish a Quality Assurance Program as required by reference (a).
- 2. Organization. The finalresponnihility for the Quality Asairance (QA) Program for Part 71 Requirements of Title 10, Code of Federal Regulations rests with the com manding Offimr, USS YELLOWSTONE (AD-41).
- a. Design and Fabrication sallnot be conducted urxler this Q A Program. The Q A Program is implimented as shown on enclosure (1).
- b. The Radiation Safety Officer (RSO) is responnihla for the overalladministration of the Q A Program including training and cerHfimtion, document control, handling, shipping, storage, inspection, test and operation status, record keeping, mntrolof purchased material, and auditing.
- 3. Quality Asurance Program. The Commanding Officer, USS YELLOWSTONE (AD-41) estahlidles and implements this Q A Program. Training, prior to engagement, for all Q A functions is required according to written procedures. - 4. Document Control Alldocumerts related to a specific shipping package willbe controlled through the use of written procedures. Alldocument changes willbe performed according to written procedures appoved by the Com manding Officer.

- a. The RSO shallensure that all QA functions are conducted in accordance with the latest apnlimhla changes to these documerts.
- 5. Handling, Storage and Shipping. Written safety procedures mnoerning the harxiling storage and shipping of packages for certain specialform radioactive material willbe followed. Shipments willnot be made unless alltests, cerHfimtions, acceptances, and finalinspections have been completed. Work instructions willbe provided for handling, storage and shipping operationc.

- 8. Audits. Etahlicthed schedulesof auditsof the QA Program wD1be performed using written check 1h Results of audits willbe maintained and reported to the RSO. Audit reports willbe evaluated and deficient areas corrected. The audits wD1be dependent upon the safety significance of the activity and willbe performed atleast once per year.
Audit reports willbe maintained as part of the quality awrance records.
