Reportable Occurrence of 14:32 on March-26th, 1987 at the MUTR Docket ~ #R-70' This letter represents the follow up report to a " Reportable Occurrence" which was reported by telephone at 15 :45 on 3/26/87.

A summary of what was reported is enclosed.

Safety channel (SC) #2 ionization chamber (IC) was moved out about Is" by mistake during n.aintenance on kW-hr compensated IC.

This error was discovered during subsequent operation, and the reactor was immediately shut down.

SC #2 chamber was repositioned and recalibrated against SC #1 by the standard procedure.

There was no significant impact upon the safety of the facility, personnel or the general public during the time interval when the SC #2 was mispositioned.

NRC April 1, 1987 The occurrence and subsequent readjustment was. discussed in a special meeting of the Reactor Safety Committee (RSC) on March 30, 1987.

The RSC approved the readjustment procedure.

It was recommended that SC #1 and #2 cable conduit be clearly marked and normal insertion position be marked in order to minimize future occurrence of this error.

SC #1 and SC #2 were marked and identified on March 31, 1987.

Summary of telephone conversation with Bob Ward, Duty Officer and Don Haberkamp, Region 1, NRC.concerning a Reportable Occurrence of March 26, 1987 at 14:32:

Maintenance Error on Safety Channel II.

1.

Called Mr. Bob Ward and Mr. Don Haberkamp of Region 1,

NRC at 15:45 on 3/26/87 to notify NRC of reportable occurrence at.14:32 on 3/26/87.

2.

Stated that the reportable occurrence has insignificant impact upon safety.

3.

Under shutdown' conditions during maintenance of the-kW-hr CIC chamber, the Safety Channel (SC) #2 was moved out'about 1/2" by mistake at 10:05 on 3/26/87.

At 14:29 on 3/26/87, while proceeding to 50kW during ENNU 320 - Reactor Operations Class, the SRO noted that the SC #2 was reading low (near zero) and at 14 :32 the reactor was manually scrammed.

5.

Stated that steps were being undertaken to readjust the position of the SC #2 so that it corresponds with SC

  1. 1 reading.

This is the standard procedure for calibrating SC #2.
