l 1.1 The final responsibility for the Quality Assurance Pro-gram for Part il requirements rests with Met-Chem Test-l ing Laboratories, of Utah. Inc.
1.2 Design and fabrication shall not be conducted under this Quality Assurance Program.

establishes and imr'.ements this Qaality Assurance Program.

The Quality Assurance Program will ensure that all defined Quality Assurance Procedures, engineering procedures and specific provistors of the package design approval are satisfitd.
The Quality Assurance Program utli emphasize control of the charac-I teristics of the pr.ckage which are critical to safety, 2.2 The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured under a Quality Assurance Program approved l
by the Nuclear Regulatory Commission for all packages designed or manufactured after January 1, 1979.
This

requirement shall be satisfied by receiving a certifica-tion to this effect frcm the manufseturer.

3.0 The Radiation Safety Officer shall ensure that all qual-I, ity assurance functions are conducted in accordance with the 1stest applicable changes to these documents.

}{ ANDI T NG, STORAGF AND SHIPPING 4.1 Written safety procedures concerning the handling, stor-age and shipping of packages for certain special form radioactive mat erial will be followed.

not be made unless all tests, certifications, necept-ances and final inspections have been completed.
Work instructions will be provided for handling, storage and

shipping operations.
4.0 Radiography personnel shall perfctm the critical handl-i ing, storage and shipping operations.

l 5.1 Inspection, test and operating status of packages for l
certain special torm radioactive material will be in-dicated and cont rolled by written ',rocedures.
Status will be indicated by tag, label, merking or log entry.
Status of non-conforming parts or packages will be positively maintained by written procedures.
5.2 Radiography personnel shall perform the regulatory re-quired inspections and test in accordance with written procedures.
The Radiation safety officer shall ensure i
that these functions are performed.

l Program will be performed using written check lists.
l Results of audits will be maintained. Audit reports will be evaluated and deficient areas corrected.
The audits will be dependent of the safety significance of the activity being audited, but each activity will be audit-ed at least once a year.
Audit reports will be mnin-tained as part o# the quality assurance records.
Mem-bers of the audit ceam shall have no rosyonsibility in the activity being audited.
