3 APPENCIX A NOTICE OF VIOLATION Cintichem, Inc.
Docket Nos. 50-54;70-687 Sterling Forest Research Reactor License Nos. R-81; SNM-639 During an NRC inspection conducted on September 6-9, 1988, violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1987), the violations are listed below:
A.
Technical Specification 6.3(4) states, in part, that procedures shall be provided for surveillance tests and calibrations required by the Technical Specifications. These procedures are required to be reviewed by the Nuclear Safeguards Committee and approved by a level 2 authority.
Contrary to the above, on September 7,1988, it was identified that some calibrations required by Technical Specifications were not being controlled by approved procedures. Among those calibrations which have not been controlled by approved procedures are the calibrations of the following instruments: power level (nominal), power level (intermediate), and count rate (startup channel).
This is a Severity Level IV Violation (Supplement I) 8.
Technical Specification 6.3(6) states, in part, that written procedures shall be provided for routine maintenance of major components of systems that could have an effect on reactor safety.
These procedures are required to be reviewed by the Nuclear Safeguards Committee and approved by a level 2 authority.
Contrary to the above, on September 7,1988, it was identified that some routine maintenance activities which could affect reactor safety were not being controlled ty approved procedures.
Among those activities which have not been controlled by approved procedures are the following: primary coolant demineralizer resin change out, primary coolant filter change out, pool cleaning, cht.nge out of rod control magnet assemblies, change out of safety amplifier :hassis, and maintenance of the primary purification pump and primary coolant pump.
This is a Severity Level IV Violation (Supplement I)
Pursuant to the provisions of 10 CFR 2.201, Cintichem, Inc. is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including: (1) the reason for the violations, if admitted; (2) the corrective steps which have been taken and the result, achieved; (3) the corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
