Purpose The ACRS Subcomittee on TVA Organizational Issues met on July 22, 1988.

The purpose of this Feeting was to discuss the generic lessons learned from the Staff's review of TVA in regard to the restart of Sequoyah Unit 2 and to discuss recent organizational changes announced by TVA, namely an overall recrganization and staff reductions. Copies of the agenda and selected slides from the presentation are attached. The meeting began at 8:30 a.m. and adjourned at 4:10 p.m., and was held entirely in '

open session. The principal attendees were as follows:

TVA Organizational Issues Meeting Minutes July 22, 1988 serious managenent breakdown throughout the TVA nuclear organization.

The initial findings by the NRC in their review of this situation were issued in SECY-E6-334, "TVA Preliminary Lessons Learned," dated November 12, 1986. Further coments en the lessons learned and the Staff's implerentatien plan for the recomendations were given in SECY-87-211 "Final Report on TVA Lessons Learned," dated August 20, 1987. In the Staff Requirerents Memorandum dated March 2, 1988, regarding SECY 211, the ACRS was requested to perform an independent review of the Staff's effort and determine if all significant lessons have been addressec. Any recom endations on this natter were requested by September 1, 1998 During June 19S8. TVA announced a reorganization plan for the corporate structure and a staff reduction plan. The Staff's review of the reorga-nization plan was issued in SECY-88-185. "Tennessee Valley Authority Technical Specificaticn Change Regarding Corporate Reorganization,"

dated July 1, 1988, The TVA reduction plans were given in a TVA Press Release dated June 29, 1958.

Discussion C Wylie. Chairran, briefly discussed the purpose of the meeting as:

(1) to review the Staff's effort on the TVA review and prepare to respond to tha request in the SRM of March 2, 1983 and (2) to determine if ary of the recent TVA organizational changes would have an adverse effect on the nuclear power organization. He indicated that a second subcomittee reeting had been tentatively scheduled for August 5,1988 and that topics for that meeting, if warranted, should be identified by the end of this meeting, E Goodwin (OSP) discussed thi Staff's effort on the review of the situation at TVA and the identification of the lessels learned as given in SECY-86-334 He also discussed the solicited coments from various individuals on SECY-86-334 His presentation provided a chronology of events, general conclusions of the review in regard to observations,

TVA Organizational Issues Meeting Minutes July 22, 1988 root causes and the NRC role, major comments on the preliminary lessons learned and the details of the recommendations for the seven lessons identified.

M. Peranich (NRR) discussed the implementation plan for the lessons learned (SECY-87-211). This was a status report on the disposition of the various recommendations. He discussed the Staff's response to an OIA review of the TVA effort (an iten in the SRM, dated March 2, 1988) and indicted that the Conmission will be provided a quarterly progress report on this effort in PRIMR. He also discussed various administra-tive documents that pertain to this effort: NRC Manual Chapter 0516 -

"Syster.atic Assessment of Licensee Performance," and two inspection Manual Chapters. 0610 "Preparation of Inspection Reports," and 2515 -

"Allocation of Resources For Inspection." He briefly described some other Staff programs in place to identify deteriorating performance and to determine appropriate regulatory action: Performance Indicators, SafetyissuesManagementSysten(SIMS)andTrendsandP4tternAnalysis.

C. Fox (TVA) discussed the corporate structure of TVA, as reorganized on July 1, 1958, and the overall effect of announced staff reductions in the Nuclear Power Group. He discu; sed the role of S. White. Senior VP, in respect to reporting to the Board and also to the Chief Operating Officer. He indicated that a Memorandum of Understanding was available which cescribed these functions, in respect to staff reductions, he indicated that there would be no significant change in TVA's ability to operate licensed plants and that it would not be acceptable to stretch out items for restart or post-start modifications at these plants. The bulk of the reductions in this Group would be at Watts Bar and Belle-fonte where the construction schedule for three units would be slipped considerably. He also discussed the nuclear safety oversight function and the role of four separate committees, groups or boards.

TVA Organizational Issues Meeting Minutes July 22, 1988 J. Bynum (TVA) discussed the organization and staffing levels for Nuclear Power Production, the specific plants at Browns Ferry and Sequoyah. He also discussed the cross-ties between the Nuclear Power Production Division with those for Quality Assurance, Nuclear Technical Director, Engineering and Construction.

, During the presentation, the following concerns, comments or requests were expressed (random order):

'l (1) INPO Reports

The Staff was asked about the availability of INPO evaluation or assessment reports which were based on a review of plant operations at Browns Ferry and Sequoyah. The Staff indicated that in accor-I dance with the MOV between NRC and INPO, copies of the reports were not provided to the NRC but the Staff had access to read the reports at IhP0 offices in Atlanta, Georgia. The Staff evalcation of the TVA situation dio not acknowledge any input from INPO and INPO was not requested to corrent on the lessons learned effort. A copy of the MOV between NRC and thPO was requested.

(2) S!MS In the SIMS process, hRC only keeps track of identified line items and their scheduled completion date. A concern was expressed about the lack of tracking the progress on these items. The Staff is only aware of schedule slippage after the scheduled completion date is passed (sometimes set to be 2-3-4 years from now).

(3) Root Cause Guidance In Lessen !!!-2, the Staff planned to develop guidance for root cause determination. It was indicated that IMC-0610 would provide some guidance on this matter. A copy of IMC-0610 was requested.

Under questioning, it did not appear that much guidance would be found in this docutent.

TVA Organizational Issues Meeting Minutes July 22, 1988 (4) Problem Plants The Staff was asked about the process used to identify problem plants or to remove them from the listing. A copy of the tran-l script of the Comission Meeting of July 13, 1988 on this matter was requested. The Staff was also asked if they use consultants in the field of nanagement review when these evaluatio".s are per-formed, it did not appear that such consultants were used.

(5) Ferferrance Indications The Staff was asked how performance indicators are used and if

their application could proside an early indication of irrpendir; problems The mini-SALP may be effective in providing this

] inferration. Perforrance indicators appear to be particularly useful for this purpose but the effort is just getting uncerway.

I (6) TVA Oreanizational Matters

) Many probing questions were asked of TVA in regard to the functions of the Senior Vice President - Nuclear Power (S. White) and the overall effect of staff reductions in the areas of plant operation, traintenance, training ar.d safety reviews. As stated above, most of the reductions were in the construction ranks due to deferrals on plants at Watts Bar and Bellefonte. Reductions were also associ.

l ated with the postponerrent of startups at Browns Ferry 1 and 3.

The TVA responses appeared to resolve any concerns expressed during the discussion.
