1.1 The following document is a description of D & S Testing, Inc.'s Quality Assurance Program for Transportation /

vackaging of Radioactive Material.

D & S Testing, Inc.

shall execute and maintain a Quality Assurance Prograin for shipping packages for transporting those quantiti.es of radioactive material licensed to D & S Testing, Inc, as required, pursuant to Section 71.51 of 10 CFR Part 71.

1 2.0 SCOPE i

material, D & S Testing, Inc.'s Quality Assurance Program shall establish those requirements for D & S Testing, Inc.

employees which will ensure:

l 2.1.1 All packages used for the shipment of radioactive material have been approved for use by the USNRC l

and are in proper condition for shipment prior 4

to any shipment being made.

l 2.1.2 All radioactive material received by D & S Testing, Inc. has been shipped in proper condition in accordance with Federal, State and 1.ocal Regulations.

1 2.1.3 Quality Control is maintained through a maintenance i

and audit program of all packages used by D & S Testing, Inc. in the transportation of radioactive

i 1.1 D & S Testing, Inc. assumes the final responsibility for the Quality Assurance Program for 10 CFR Part 71 requirements, I

1.2 The Design and Fabrication of radioactive material shipping packages shall not be conducted under this j

Quality Assurance Program.

1.3 The Quality Assurance Program is implemented in conjuction with the organizational chart.

(Attachment #1) 1.4 The Radiation Safety Officer shall be responsible for assuring compliance with the Quality Assurance requirements i

of Part 71 and for overall administration of the Quality 4

Assurance Program.

IIis duties shall include, but not bc l

limited to responsibility for training, certification, document control and auditing.

I 1.5 Radiographers shall be responsible for handling, storing, j

shipping, inspection testa, opereting status, and record i

keeping in accordance with extablished procedures.

2.0 QUALITY ASSURANCE PROGRAM i

2.1 The management of D & S Testing, Inc. establishes and implements this Quality Assurance Program.

2.2 Trnining for all Quality Assurance functions, prior to engagement in these functions, is required in accordance with written procedures.

2.5 The Quality Assurance Program will emphasize control of the cha rac te ri s t ic s o t' the package which are critical to safety.

2.5 The Radiation Safety Officer shall assure that all radio-active material shipping packages are designed and manufactured under a Quality Assurance Program approved i

by the Nuclear Regulatory Commission for all packages designed or fabricated after July 1st, 1978.

This requirement can be satisfied by receiving a certification l

to this effect from the manufacturer.

i 3.0 DOCUtiENT CONTROL 3.1 All documents related to a specific shipping package will be controlled through the use of written procedures.

All document changes will be performed according to written procedures approved.by management.

i 3.2 The Radiation Safety Officer shall ensure that all Quality Assurance functions are conducted in accordance with the latest applicable changes to these documents.

i 4.0 IIANDLINr STORAGE AND SillPPING O

4.1' Written safety procedures (OEM Section RS-GP-3) concerning the handling, storage and shipping of packages for certain special form radioactive material will be followed.

i 4.2 Shipments will not be made unless all tests, certifications, acceptances, and final inspections have been completed.

4.3 All handling, storage, and shipping shall be conducted in accordance with written work instructions.

5.0 INSPECTIONS, TESTS, AND OPERATING STATUS I

5.1 Inspections, tests and operating status of packages for i

certain special form radioactive material will be indicated l

and controlled by written procedures.

Status will be

indicated by tag, label marking or log entry.

l 5.2 Status of non-conforming parts or packages will be positively maintained by written procedures, i

i i-5.3 Only those persons, qualified as radiographers, who are

trained and certified in the applicable test methods shall perform required inspections and tests, in accordance with written procedures.

l 5.4 It shall be the responsibility of the Radiation Safety l

Officer to ensure these functions are properly performed.

6.1 Records of package approvals (including references and drawings), inspections, tests, operating logs, audit results, personnel training, and qualifications, and records 1

of shipments will be maintained.

Descriptions of equip-ment and written procedures will also be maintained.

l 7.0 AUDITS i

j 7.1 Established schedules of audits of the Quality Assurance l'rogram will be performed using written check list.

The results of audits vill be maintained and reported to management.

i 7.2 Audit reports will be es.iluated and deficient areas corrected.

l 7.3 Audit frequency shall depend on the safety significance of the activity being audited, but each activity will be audited at least once per year.

Auditing of proper 1

handling, storage, and shipping shall be included in l

Radiographers Quarterly Safety Auditing, i

7.4 Audit reports will be maintained as part of the Quality l

Assurance records, i

7.5 Members of the audit team shall not have the immediate i

responsibility for the activity being audited.
