  • e NOTICE OF VIOLATION Culf States Utilities Decket No. 50-458 River Bend Station License No. NPF-47 EA 86-74 During an NRC inspection conducted on March 17-20, 1986, violations of NRC requirements were identified. The violations involved a failure to meet operability requirements for the Division II Emergency Diese) Cenerator IB, a failure to perform maintenance in accordance with procedures, and inadequate c6rrective action for a previous emergency diesel generator operability problem.

In accordance with the " General Statement of Policy and Procedure for NRC

. Enforcement Actions," 10 CFR Part 2 Appendix C (1985), the violations are listed below:

A. Technical Specifications, paragraph 3.8.1.1, requires that three independent emergency diesel generators be operable (IA, IB, and IC) when th'e plant is in Operational Conditions 1, 2, or 3.

Contrary to the above, between February 17, 1986 and March 17, 1986, during which the reactor was in Operational Condition 1, 2, or 3, only 2 of 3 independent emergency diesel generators were operable in that Diesel Generator IB had a mispositioned fuel system filter stop valve which resulted in a loss of fuel oil flow to the diesel.

B. Technical Specifications, paragraph 6.8.1, requires that written procedures be established, implemented, and maintained for the conduct of maintenance on safety related equipment.

Licensee Procedure ADM-028, " Maintenance Work Request (MWR)," Revision 4, requires an.MWR for maintenance on plant equipment so that actions, necessary authorizations, work methods, conditions required to perform maintenance, and conditions for returning the component'to service are documented.

Contrary to the above, on February 14, 1986, an MWR was not used for the removal of the fuel filter valve handle from the fuel filter stop valve for Emergency Diesel Generator IB which is safety related. In addition, an MWR was not used for the installation of a new handle on February 17, 1986.

C. 10 CFR Part 50, Appendix B, Criterion XVI, requires that measures be established to assure that conditions adverse to quality are promptly

~1denlTfied and corrected. The approved Qualicy Assurance plan as described in Chapter 17 of the Final Safety Analysis Report for River Bend commits to this requirement, and licensee Procedure ADM-0019,

" Initiation and Processing of Condition Reports," Revision 3, is a method by which this is implemented. Procedure ADM-0019 requires that conditions adverse to quality be described fully and that remedial action to correct generic problems be described.

s. e Notice of Violation Contrary to the above, corrective actions documented in Condition Report 86-0179 for Diesel Generator IA did not include generic corrective actions for the fuel filter stop valve after it was found to be mispositioned on February 14, 1986 with the handle missing.

Collectively, the above violations have been evaluated as a Severity Level III problem (Supplement I) .

Pursuant to the provisions of 10 CFR 2.201, Culf States Utilities is hereby thquired to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each viciation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
