The events discussed and the significant elements of these events are presented in Enclosure 2. Enclosure 3 presents a sumary of reactor scrams. Three significant events were identified for input to NRC's perfomance indicator program.

88-42 LIMERICK UNIT 1 FUEL LEAK PROBLEMS OCTOBER 13, 1988 EEDhLEB THE CONTROL ROOM EMERGEkCY FRESH AIR SYSTEM (CREFAS) RECEIVED A VALID HIGH RADIATION SIGNAL WHILE THE CREFAS WAS BEING MANUALLY OPERATED IN THE CHLORINE MODE.

ChkSE FAILED FUEL IN UNIT 1 IS RELEASING ITS GAP ACTIVITY IN THE FORM OF NOBLE GASES AND 10 DINE WHICH ARE ESCAPING THE RCS THROUGH STEAM LEAKS.

SAFETY SIGNIFICAhff THIS IS Af! EVENT OF INTEREST.

DJSL1)SS10ll o LIMERICK IS NOW OPERATING AT 57% POWER, AFTER SEVERAL VOLUNTARY POWER REDUCTIONS DUE TO FUEL PIN (6-16) FAILURES WHICH BEGAN IN MARCH 1988.

o CATASTROPHIC FUEL FAILURE IS UNLIKELY.

o SOME FUEL BUNCLES IN THE CORE CONTAIN NON-HEAT TREATED ZlRCALOY CLADDI!!G. IN THE PRESENCE OF HIGH CONCENTRATIONS OF COPPER, THIS NON-HEAT TREATED FUEL CAN DEVELOP CLADDING DEFECTS WHICH ARE l BEllEVED TO BE CAUSED BY CRUD INDUCED LOCALIZED CORROSION (CILC).

l 0 CILC CANNOT BE CONFIRMED UN11L THE FUEL IS REMOVED BUT SIMILAR PROBLEMS llAVE OCCURRED AT OPERATING PLANTS WITH NON-HEAT TREATED ZlRCALOY CLADDlhG !!! THE PRESENCE OF HIGH (GREA1ER THAN .5 PPB)
