QUAD CITIES INSPECTION REPORT 50-254/99003(DRS); 50-265/99003(DRS);

Dear Mr. Kingsley:

This refers to the inspection conducted March 8 through 12,1999, at the Quad Cities Nuclear ~

Power Station. The purposes of the inspection were to evaluate the program and procedures

regarding maintenance of electrical power circuit breakers, to assess corrective actions for-

previously identified maintenance problems, and to review maintenance related materiel-

condition problems.

The circuit breaker maintenance program and procedures appeared acceptable with a few

minor exceptions related to 480V breakers. Corrective actions, in general, were appropriate and

timely. Self assessments were thorough and sufficiently self-critical. Longstanding materiel ~

condition issues continue to be a problem; however, progress had been made in some areas,

such as improving the reliability of reactor recirculation pump seals and recent management

initiatives to make restoration of offgas trains for both units a priority.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosure will be 'placed in the NRC Public Document Room.

The circuit breaker maintenance program and procedures appeared to be acceptable with a few

minor exceptions related to the 480V breakers, such as, a lack of minimum voltage testing, lack

of "as found" data in refurbishment reports, and a lack of documentation to support seismic

qualification of 480V switchgear with breakers removed. Corrective actions, in general, appear

to be appropriate and timely. Self assessments were thorough and appeared to be sufficiently

self-critical. Longstanding materiel condition issue continue to be a problem.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclor.ure will be placed in the NRC Public Document Room.
