Dear Ms. Sakimoto:
Notice is hereby provided that your Quality Assurance (QA) Program Approval for Radioactive Material Packages, Approval No. 0897, expired on August 31,1998.
If you are an NRC licensee and conduct c::tivities under the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement State Licensee subject to the requirements of 10 CFR Part 71, as required by 10 CFR $ 150.20, a QA Program approved by the Commission as satisfying the provisions of Subpart H of 10 CFR Part 71 is required. Since you no longer have a valid QA Program Approval, the General Licenses of Subpart C of 10 CFR Part 71 and registration under the provisions of 10 CFR S 71.12, to transport or deliver licensed material to a carrier for transport are terminated.
Please note that records required by 10 CFR 99 71.91 and 71.135 are to be retained for a period of three years.
