FERMI STATUS FOR THE PERIOD SEPTEMBER 1, 1988 - SEPTEMBER 30, 1988

l. P l a_n_t S t a t u s As of 0800 on September 30, 1988, the plant was in cold shutdown pending NRC approval for restart following an investigation of the failure of the "B" reactor recirculation loop discharge valve to close when signaled to do so from the control room.
2. Facility Operations Summary The plant entered September in cold shutdown following the repeat event in which the pump discharge valve in reactor recirculation loop B failed to close from the control room. Investigation into this event revealed a generic problem regarding the adequacy of the licensee's control of MOV torque and limit switch settings. The licensee remained shut down throughout the month of September, under the August 30, 1988 Confirmatory Action Letter discussed in the previous status report. In a management meeting on September 27, 1988, the licensee presented the results of its investigation, and subsequently submitted its written response to the Confirmatory Action Letter on September 28, 1988.
3. Items of Special Interest Control Room Eyclution Evaluation Program - The licensee :ontinued ta perform assessments of control room activities during the reportir.g period.
4. Change to MonthL Status Reporting This report will continue to be issued monthly with the next report due on November 10, 1988.
5. NRC Staff Activities During the Period The resident inspector staff consisted of the senior resident inspector (Walt Rogers) and the resident inspector (Stan Stasek).

On August 15 - September 9,1988, an inspection was conducted by one DRS inspector to review (1) the licensee's imple wntation of Generic Letter 83-28 in the areas of equipment classificati ., vendor interface, post maintenance testing, and reactor protection system reliability; (2) Bulletins; and (3) open items. One violation was identified regarding inadequate procedures to control vendor equipment technical i n fo rma t ion.

On September 6-21, 1988, a special inspection was conducted by two DRS inspectors to review the licensee's corrective actions in response to the repeat event in which the pump discharge valve in reactor recirculation loop B failed to close from the control room. The inspection report is currently in draft.

o The 19-member Otagnostic Evaluation Team completed the last week of

, ~bnsite inspection during the period of September 12-16, 1988.

On September 19-20, 1988, a DRS inspector conducted an onsite preparatory visit for the two-week, five member Engineering Team Inspection to be conducted in October and November.
