Inspection Report No. 50-309/86-20 Gentlemen:

This letter is in response to Inspection Report No. 50-309/86-20.

The cover letter to your inspection report required Maine Yankee to inform you of any measures that were being taken to prevent an incident, similar to the inadvertent release that occurred on December 30, 1986, from occurring.

Our investigation of the incident determined the root cause of the occurrence to be the improper installation of a blank flange on a nitrogen feed line to the Volume Control Tank. After refuelings a spool piece is removed from the nitrogen feed line ar.d a blank flange is installed to prevent inadvertent feed of nitrogen to the tank during normal operation.

It was normal practice to have Operations personnel remove this spool piece and install the blank flange.

To prevent the occurrence of similar incidents, the Operations Department procedure calling for this evolution to be performed has been revised to require Operations to request the work be done by the Maintenance Department utilizing the management controls which Maine Yankee uses for this type of maintenance work, including post maintenance leak checking.

A leaking check valve, upstream of the blank flange, was identified as a 2

contributing cause of this incident. The check valve has been scheduled for inspection and repair during the upcoming outage.

He believe these actions are sufficient to prevent recurrence of a similar 1
