Pursuant to Technical Specification 6.9.2 and the requirements of Technical Specifica-tion 3.3.3.7, we have attached a Special Report for inoperable fire detection instrumentation in the Unit 2 Service Water Pump Room. The Special Report outlines the action we have taken, the cause of the inoperability and the corrective actions to prevent recurrence.

SPECIAL REPORT PURSUANT TO TECilNICAL SPECIFICATION 3.3.3.7 UNIT TWO SERVICE WATER PUMP ROOM INTRODUCTION The flame and smoke detector circuit for the Unit Two Service Water Pump Room was discovered defective during the performance of Surveillance Test hi-490-0, Fire Detection Functional Test, on September 23, 1988. The instruments were restored within the 14 days specified by Technical Specification 3.3.3.7(b). Ilowever, a wiring error was discovered in the detector circuit during maintenance of the circuit. Therefore, i
we have prepared this Special Report.
ACrlON TAKEN On September 23, 1988, the required actions of Technical Specification 3.3.3.7(a) were p
taken after the defective circuit was discovered. The circuit was repaired within 13 hours1.50463e-4 days <br />0.00361 hours <br />2.149471e-5 weeks <br />4.9465e-6 months <br />. During maintenance, two leads were discovered reversed in the local junction box OPS /OZS 205 of the Service Water Pump Room. An investigation was conducted to determine whether any maintenance was conducted on the junction box since the last successful completion of Surveillance Test ht-490-0 on htarch 16, 1988.
CAUSE OF INOPER ABILITY The flame and smoke detector circuit in the Service Water Pump Room is polarity sensitive and will not alarm as designed when the positive and negative leads are reversed. The circuit trouble alarm will operate; the flame and smoke detector circuit alarm will not operate. Surveillance Test hi-490-0 was performed on September 23, 1988, and failed. Subsequent maintenance determined that the positive and negative leads were reversed.
A review of fire system impairments and maintenance of this circuit was conducted for l
the period since the last successful completion of Surveillance Test ht-490-0 on l
htarch 16, 1988. hiaintenance was performed in the local junction box on June 17, 1988, but no details of it were documented. Discussions with the technicians indicated that I
no leads were lifted or discovered loose in the local junction box.
While the cause for the leads being resersed appears to be due to maintenance performed In the local junction box on June 17, 1988, this cause cannot be substantiated.

The consequences of apparently having no flame or smoke detection alarm for the Service l
Water Pump Room, since June 17, 1988, are minimal when the following is considered:

The Wet-Pipe Fire Suppression System for the Service Water Room remained l
operable since the Surveillance Test performed on htarch 16, 1988.

o Any fire in the room would be detected by the actuation of the Fire i
Suppression S.vstem through a local water-flow-operated gong and an alarm
[
check valve actuation ' Fire" alarm in the Control Room. The Control Room t
alarm would identify the fire to be in the Service Water Pump Room.

o While the hourly fire patrols required by Technical Specifications 3.3.3.7(a) were not performed, a plant operator and a security person each entered the Service Water Pump Room at least once every four hours.

CORRECTIVE ACrlONS TO PREVENT RECURRENCE o
The esent will be reviewed with all Electricians. Electricians will be made aware that the smoke and flame detectors are polarity sensitive; that.
except in an emergency, no work will be performed on a safety-related system, the fire system or any other system which can affect Technical Specifications without the use of approved maintenance documentation; and that, all work will be documented by the end of the shift.
o Electrical Maintenance Planners will be instructed to be more specific in delineating job instructions and post-maintenance testing requirements.
Troubleshooting boundaries will be specified, o
Unit Supervisors will be instructed to critically review job instructions /
post.naintenance testing to ensure proper guidance and boundaries are set.
Additionally, if the maintenance is conducted on a system which affects Technical Specifications, the appropriate surveillance test will be performed to verify operability.
