There are three levels of certification, Level I,
Level II and Level" III.
Level I
inspectors are basically trainees without the authorization to interpret examination results.'
Level II inspectors are authori:ed to administer examinations and interpret the results of those examinations.
Level III personnel administer NDE activities on the discipline or program level.
They write procedures, interpret requirements (codes and standards) and monitor / administer the training anc certification program for NDE insoectors.
Eecause the Level I
inspectors do not interpret examinations and level III personnel do not normally perform them, the training file review was directed primarily at the Level II certifications.
A visit was made to the pOTC for the purpose of reviewing NDE certification / training files.
pOTC NDE Supervisor (name known) allowed the review of the training files maintained by his department. He asked that he be informed of any discrepancies noted during this file review.
This was done.
A total of sixteen (16) ISI inspector files consisting of forty-three (43) NDE certifications and twenty-four (24) site GC inspector files consisting of aighty-two (82) NDE certifications were reviewed.

forms and 2)
Determine the degree of compliance to TVA procedures for training / certification f { 1es.
Item #1 A
QC Superv,1.sor (name known) provided a current list of QC personnel performing NDE inspections at SONP.
From this list,. a chart was compiled to reflect the amount of Job training for each.idspector (per the exhibit "D")
for 1983.
1983 was selected because the majority of SONP_OC inspectors were certified in that year.
After completing the chart, a review was made of the DC inspection log (which is required by AI-20) to determine the number and typc inspection performed by each inspector.
This information was added
'o the chart.
The disciplines reviewed were VT, PT, and MT.
It was not possible to maintain an accurate chart on VT because the DC inspection log failed to denote recorded inspections asSection III or Section XI.

The site QC group submitted to
- POTC, exhibit "D"
forms which indicated seven (7) months of MT had been performeo at SQNP during 1983, the QC inspection log revealed three (3) inspections performed in this discipline during that year.
There exists the possibility of an error factor due to difficulty in reading anc uncerstancing the casually maintained inspection log, however, the error factor could not cover the stated seven (7) months of MT inspections.

revicw was made of the training files for all forty (40) inspectors employed by ISI (16) and SONP GC (24).
Attention was directed toward NDE certifications for Level II inspectors.
It is not practical to list all discrepancies noted during this review due to the repetition and large
- numbers, however, the following are examples of those discrepancies:

Eye examinations which expired without the certification (s)

- POTC personnel (names known) were. questioned regarding certification" dates which were earlier than test sheets and exhibit "D"
forms.
They responded by stating the certifications were not' signed until after all recuired documents were available, reviewed and accepted.
They continued with the statement that the date typed on the certification sheet was t,he date the incividual was " certifiable",
even though the testing may not have been completed on this date.
They also pointed out that the certification date was certainly not the date the certification was signed.
When asked for data / documents to support the actual date the certification was signed, none could be provided.
In addition to the numerous discrepancies noted in the training files, it is imoossible to determine exactly when a NDE certification begins, j
TVA has been made aware of difficulties within the NDE I
training / certification program on several occasions.

INPO Evaluation - page #16 (April, 1985) Audit MSG-83TS-05 (3-14/31-83) and ANII Report #15 (6-11/26-85) have all identified unique areas of the training / certification which required corrective action.
Even though no comprehensive study of the NDE certification program can be located, POTC has generated a preliminary document to rqsolve the majority of problems documented in this report.

" Quality Control and Nondestructive Examination On-The-Job Training Manual" was originally projected for issue in early
- January, 1986, however, several areas of the manual will require revision The statement in the DJT
- manual, "The DJTL. Qualification Guide...does not require implementation",
dodn~ grades this document from a requirement to a guide.

This document doe's not state who is within its scope or who will be subject to its implementation.
Section II. B (2) (6) does not define the term "... based on...".
The manual fails to address what actions will be taken if a
trainee fails some portion of the manual.

DETAILS, continued The logic behind the ASNT requirement for work time experience was' Questioned.
Every inspector was asked the following question:
"Can an individual with no prior experience in NDE, pass the POTC course (in any discipline)".
Not one inspector answered yes.
Every inspector felt the work place was where the discipline was learned.
With this importance attached to work time experience, the necessity for monitoring / documenting this aspect of certification becomes apparent.
CONCLUSION:
This concern is substantiated.
When discrepancies were noted during this investigation, and were shown to the individuals responsible for these
- items, there was always a " story" as to why the situation existed.
These
" stories" were never supported by procedure or objective evidence.
Therefore, the facts only must be used to form conclusions.
These facts include the following:
A very high percentage of interviewed NDE inspectors feel that exhibit "D"
forms were not correct or truthful (theirs and others).
See the NSRS report.

The lack of definition in explaining exactly what the wording in the exhibit "D"
form means.
The ~high percentage of exhibit "D"
forms wh,ich cannot be supported by objective evidence.

The failure by TVA to - withdraw certifications wh *+ n eye examinations are not taken (therefore, not passed) as required by O202.14 RECOMMENDATION:
The results of this investigation clearly indicates both a programmatic breakdown and falsification of records within the TVA NDE training / certification program.
Based on these findings, the following is recommended:
1.
The turn over of this report to the Office of General Counsel (OGC) for investigation of legal wrong doing, and 2.
TVA issue an immediate stop work order against the certification of NDE inspectors until such time as the situation can be evaluated and corrective action taken.

Certification sheets reflect signature dates which are different from the certification dates.
Even though statement
- 6 states "...is hereby certified to the level listed above",
the earlier (typed) date is used for the actual date of certification.

Cert i.f,1 cat ion dates are typed on the record of certification.
Examples were noted where test sheets were signed and dated after this certification date.
Because the test' sheets are required for certific^ation, it is clear that certifications were signed and authorized without all required criteria being met.

DPM N75C01 requires anyone failing one section of the test to retake all three sections.
This was not'done.

There are three basic criteria for certification in the area of NDE:
- 1) Current eye examination,
- 2) Documented evidence of work time experience, and 3) Successful completion of the formal training classes.
Examples were noted where the documented evidence of work time experience is dated sometime after the certification was issued.
It appears that the certification was signed without all required criteria being 1
met.

VISUAL WELD EXAMINATIONS (SECTION III)
A review of the Visual Weld Examinations (Section III) which were administered prior to April 28, 1983 has indicated that some of the practical examinations were not in full compliance with the requirements of Note 5 Page 23, of
.DPM N75C01, dated October 14, 1982. Based on this, the following corrective -
action has been initiated.
The certifications of all personnel possessing a discrepant practical 1.
have been revoked ef fective either the date of their first examination or May 1, 1983 in accordance with OQAM Part II, Section 5.3.A.-
Personnel will be recertified af ter successfully completing a new 2.
practical examination. This recertification shall be reinstated ef f ective to the revocation date established in (1) above.
