APPENDIX A NOTICE OF VIOLATION Combustion Engineering, Inc.
Docket No.
70-1100 Windsor, Connecticut 06095 License No. SNM-1067 As a result of the inspection conducted on December 9-13, 1985, and in accord-ance with the NRC Enforcement' Policy (10 CFR 2, Appendix C), 49 FR 8583 (March 8, 1984), the following violation was identified:
A.
Section 7.4, " Tamper-Indicating", of your NRC approved Fundamental Nuclear Material Control Plan, Revision 5, dated February 1983, states, in para--
graph 7.4.7, " Precise records of each and all seals by serial number will be kept and, after application, will include data on the sealed contents.
These records will include the seal number, item being sealed, date and time of application or disposal of. the seal, the signature of the.individ-ual responsible for applying and removing the seal, and any discrepancy that is observed in the sealed contents."
Contrary to the above, on December 11, 1985, the inspector noted that between the period of November 11, 1985 and December 11, 1985, the records of the time of application of seals and the signature of the individual responsible for applying the seal were not kept.
This is a Severity Level V violation (Supplement VI E).
Pursuant to the provisions of 10 CFR 2.201, Combustion Engineering, Inc. is hereby required to submit to this office within thirty days of.the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which' have been taken and the re-suits achieved; (2) corrective steps which will be ~taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
