SUBJECT:

STATUS OF DEVELOPMENT FOR THE INCIDENT INVESTIGATION PROGRAM On November 14, 1985 I was directed by the ED0 to develop the necessary guid-ance for an event investigation program (please see attached). Specifically, I was directed to take six actions (discussed below) and a target date of July 1986 was established for the completion of this development. The six specific tasks identified by the ED0 are repeated below together with the status as of July 31, 1986.

1.

Prepare an NRC Manual Chapter that will define the duties, responsibili-ties and schedule for event investigation under the Incident Investiga-tion Program. This Manual Chapter is to contain guidance regarding the significant operational events to be investigated by an Incident Investigation Team (IIT) and those warranting an augmented response by Headquarters personnel.

Status: Completed. The Manual Chapter was prepared, concurred in by all offices and forwarded to the ED0 for approval on July 31, 1986.

2.

Prepare personnel rosters of candidate IIT leaders and members so that an IIT can be promptly established. These candidates should be preapproved by the Office Directors on the basis that if the individual is selected for IIT duty, he or she will be automatically relieved from existing assignments.

Status: Completed. Personnel rosters for candidate IIT leaders and members have been prepared, reviewed with all offices, and distributed for information and use as appropriate. A master roster is maintained by AE00 and an updated roster, approved by the EDO, will be distributed semi-annually.

Develop appropriate training plans for candidate IIT leaders and members and provide assistance for arranging for such training to be conducted as soon as possible.

Status: Completed. A two-week training program covering event investiga-tion methodology and the IIT procedures was developed, and given to 25

representatives from Headquarters and Regional Offices on July 7-18, 1986. This class composition was selected on the basis of five teams.

4.

Prepare supporting procedures covering IIT activities. These procedures are to include the specific points and concerns identified in the Connis-sion Paper.

Status: Completed.

IIT procedures have been prepared, reviewed by involved offices, discussed during IIT truning, and issued for trial use and comment.

These procedures will be furdar reviewed by industry organizations, discussed in regional workshops and issued in final form in December 1986.

Work with ELD to draft suitable language and procedure'; for issuing (and removing) an immediately effective Order or Confirmatory Action Letter requiring that, within the constraints of maintaining plant safety, equipment is left in the "as found" condition and information and data concerning the event are retained.

Status: Completed. Suitable language and procedures for issuing an Order or Confirmatory Action Letters have been incorporated into the IIT procedures (discussedinItem4above).

Investigate the need for and feasibility of providing the capability to invite representatives from outside the NRC (e.g., INPO, NSSS suppliers) to participate in the IIT investigation; to providing subpoena power to the IIT; and to providing the authority to preempt parallel investiga-tions by other organizations if they interfere with the IIT investigation.

Sta tus: Ongoing. An IIP objective is to have outside representation.

Towards this end, an INP0 representative participated in the IIT process for Rancho Seco. Routine participation as a team member in future IITs will be discussed with industry organizations as part of the forthcoming regional workshops. The. subjects of the need for subpoena power by the IIT and authority to preempt parallel investigations are subjects still under review with OGC. These latter two items should be resolved in the schedule specified in the IIP Program Plan (i.e., in September 1986).

The program plan covering these and other IIP development activities was prepared, reviewed by other offices and issued in final form in December 1985 as requested in the November 14, 1985 memorandum from the EDO.

In summary, the principal milestones contained in the ED0 guidance have been accompiished for reactor events.

SUBJECT:

IMPLEMENTATION OF A REVISED PROGRAM FOR THE INVESTIGATION OF SIGNIFICANT OPERATING EVENTS In order to ensure that the investigation of significant events is structured, coordinated and formally administered, AE00 is assigned responsibility for developing and managing the Incident Investigation Program. You are requested to develop the necessary guidance for this expanded program of event investiga-tion. This guidance is to be consistent with the commitments and characteris-tics of the revised program for the investigation of significant events as defined in my paper (SECY-85-208) to the Conrnission dated June 10, 1985 and approved by the Commission on October 16, 1985.

Specifically, you are requested to develop and implement a program plan that includes the following activities:
