PLANT PERFORMANCE REVIEW (PPR) - COMANOHE PEAK STEAM ELECTRIC STATION

Dear Mr. Terry:
On February 11,1999, the NRC staff completed a PPR of Comanche Peak Steam Electric Station. The staff conducts these reviews for all operating nuclear power plants to develop an integrated understanding of safety performance. The results are used by NRC management to facilitate planning and allocation of inspection resources.' PPRs provide NRC management with a current summary of licensee performance and serve as inputs to the NRC's senior management meeting (SMM) reviews. PPRs examine information since the last assessment of licenses performance to evaluate long-term trends, but emphasize the last 6 months to ensure that the assessments reflect current performance. The PPR for Comanche Peak Steam Electric Station involved the participation of all technical divisions in evaluating inspection results and safety performance information for the period June 8,1997, through January 25, i
1999. The NRC's most recent summary of licensee perforrnance was provided in a letter of July 7,1997, and us discussed in a public meeting with you on July 30,1997.
As discussed in the NRC's Administrative Letter 98 07 of October 2,1998, the PPR provides an assessment of licensee performance during an interim period that the NRC has suspended its Systematic Assessment of Licensee Performance (SALP) program. The NRC suspended its SALP program to complete a review of its processes for assessing performance at nuclear power plants. At the end of the review period, the NRC will decide whether to resume the SALP program or terminate it in favor of an improved process.
At the beginning of the current period of detailed focus (April 22,1998), Unit 1 was in its sixth refueling outage. On April 28, the output breakers were closed, completing the 38-day outage.
During the return to power, a heater drain pump developed high bearing temperature and was g'
shut down. Bearing replacement delayed achieving 100 percent power until May 4. Unit 1 l
remained at 100 percent reactor power for the remainder of the assessment period.
Unit 2 began the current period of detailed focus at 100 percent reactor power. On July 16, reactor power was reduced to approximately 80 percent in response to high vibrations on the low pressure turbine blades which were induced by high circulating water temperature. On September 12, power was returned to 100 percent when circulating water temperature and the associated turbine blade vibrations decreased. TU Electric equivalent 80 percent loss of load and the operators manually tripped the unit. After replacing circuit cards in the load reference logic circuit, the unit was restarted on January 5 and returned to 100 percent power on January 6.
Overall performance at Comanche Peak has continued to be acceptable. Performance in the operat;ons area exhibited a strong safety focus and conservative decision making. The maintenance area was characterized by consistent performance and excellent plant material condition. An overall strong Maintenance Rule program has been developed. In the engineering area, isolated design problems posed challenges to operators during transients.
Corrective actions were generally appropriate and conservative, but examples of weak troubleshooting were recently identified. Performance in the plant support area continued to be effective.
Performance in the operations area co,tinued to demonstrate a strong safety focus, effective command, control, and communications, and conservative decision making. Operator performance during both routine and abnormal operations continued to be strong; however, occasional errors occurred involving lack of attention to detail. Occasional errors in plant

conf.guration control had little or no safety significance. Operators' knowledge level continued t
to be good, although a lack of understanding of the impact of loss of extraction steam on plant efficiency resulted in exceeding the administrative limit on reactor power ramp rate. Applicants for operator examinations were well prepared; however, test materia 1 provided by the. licensee for the administrative topics and control room systems and facility walk-through was inadequate for administration. Only core insper.tions are planned for the next 8 months.
J Maintenance, surveillance, and troubleshooting activities were well performed and a j
i conservative approach towards maintenance was evident. Good coordination and communications were noted; however, some coordination problems during refueling outage activities occurred. Plant material condition continued to be excellent, equipment reliability

improved, and the maintenance backlog for some safety-related systems decreased. Strengths
)
and positiv'e findings associated with the Maintenance Rule included the management support of the overall program, the effective self-assessment capabilities, conservative action level j
criteria for reliability and unavailability, and the categorization of the safety significance of systems, structures, and components. Maintenance Rule program weaknesses were not J
significant. Only core inspections are planned for the next 8 months.
Engineering performance included one instance of an inappropriate use of a maintenance procedure to install temporary instruments, a condition which resulted in the partial loss of reactor vessellevelindication. Otherwise, engineering support to assure safe plant operations j
was good. The response to NRC letters and notifications was thorough and extensive.
Implementation of the corrective action process by engineering was good; however, troubleshooting activities were not always effective. Some long-standing system performance problems continue to pose a challenge to operators during transients. Only core inspections are needed to monitor performar.ce over the next 8 months.
The radiation protection program was effectively implemented. The plant was maintained very clean radiologically and was appropriately surveyed and posted. Radiation work permit requirements were known by workers. The chemistry department continued to aggressively pursue and restore plant chemistry to high standards. The emergency preparedness program

TU Electric 3-was effe^tive. Relocation of the emergency operatiens facility to the alternate site was -
proble mic early on, but subsequent improvements were noted. A recent inspection found that the entergency preparedness program was generally well implemented and significant improvement was _noted during the simulator walkthroughs. Performance in the security area continued to be good, but isolated access control issues were noted. Only core inspections are planned for the next 8 months, with the exception of the Operational Safeguards Response

Evaluation Safety inspection. contains a historical listing of plant issues,' referred to as the Plant issues Matrix (PlM), that were considered during this PPR process to arrive at an integrated view of licensee performance trends. The PIM includes items summarized from inspection reports or
- other docketed correspondence between the NRC and TU Electric. The NRC does not attempt to document all aspects of licensee programs and performance that may be functioning appropriately. Rather, the NRC only documents issues that the NRC believes warrant management attention or represent noteworthy aspects of performance. In addition, the PPR
. may also have considered some predecisional and ciraft material that does not appear in the attached PIM, including observaticns from events and inspections that had occurred since the last NRC inspection report was issued but had not yet received full review and consideration.

This letter advises you of our planned inspection effort resulting from the Comanche Peak Steam Electric Station PPR review, it is provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Enclosure 3 details our inspection plan for the next 8 months. The rationale or basis for each inspection outside the core inspection program has been provided in this letter so that you are aware of the reason for emphasis in these program areas. Resident
